The well was fenced and locked. The last reported production for this well was in 2015. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The vent stack on the production tank was plumbed into a 55 gallon charcoal filter then plumbed up and topped with a spark arrestor. Tank valves were equipped with flat plugs. The tank battery was diked and fenced with a locked man door. The wellsite did not have any signage associated with it, so I contacted Mark Turpak from Cutter about installing signage. The lease access was stable. |

Annulus has a pop off on it, annulus gas is vented through the charcoal filter in the tank battery. Spacing between the production tanks is 5 inches too close. Spacing between the production tanks and the separator is correct. Tank battery fence only has a single door. Affected area has been reclaimed. Lease road is stable. No gate on lease road, drive is also a primary driveway. There is erosion on the bank at the stream crossing. Bank of the stream crossing needs repair seeding. Operator has recently completed repair work on the stream crossing. Company will be contacted by e-mail regarding need repairs. |

The lease road is in stable condition. |
