0571908293 2018-05-08 Final Restoration No Violations I inspected this well location as an evaluation for final restoration. According to documentation, the well was plugged and abandoned in 2008 and a plugging report was submitted. At the time of this inspection, I found the former area of the well to be restored, with well-established grasses. There were no signs of the former well or associated production activity. I did note that the presumed gravel-paved lease road remained in place. I will confirm with Bass Energy and the landowner whether the lease road is still present and whether the landowner wished to waive the resotration. Until that time, Final Restoration will remain pending.
1219421948 2008-08-22 Drill / Deepen / Reopen No Violations Rig is moving on site. Working pit has been completed and liner installed. Liner has slack in it, no tight areas. Soil is loamy, mix of sand and clay. 2nd pit is under construction. Drainage controls have been installed, silt fence is up around site. Silt fence is installed correctly. Lease road is stable, constructed out of durable material. Permit ID sign is posted at the entrance of the site.
1219673744 2008-08-25 Drill / Deepen / Reopen No Violations Rig is drilling in the shale on air, drill depth 1370. Pits are in good repair, liner is up on the back stop, to prevent dusting. Lease road is stable. Permit is posted. Drainage controls are in place.
1221567101 2008-09-15 Plug / Plug Back No Violations Operator is plugging well this day. Operator was bailing frac fluid out of the well bore to prepare formation for cement. Bottom plug set with 50 sacks of cement. Silt fence is up around site, small area west of well needs repaired. Lease road is stable. Blow pit liner has slipped operator was actively pumping pit down at the time of inspection.
1221657415 2008-09-16 Plug / Plug Back No Violations Pipe ripped at 2735 operator pulled 11 joints to 2251. Operator broke circulation ran cement. Lime plug set 2251 to 1951. Operator pulled 42 joints. Broke circulation, ran cement. Surface plug set 396 to surface. Cement returned to surface. Operator pulled last 9 joints of pipe. Cement was down 5 from surface. Operator will cut off pipe and reclaim site. Pits will need to be closed as soon as possible.
1720059977 2008-08-27 Drill / Deepen / Reopen No Violations Drill depth 3250 operator drilling on air. Operator constructed pit with a back stop, pit liner goes up the side of the back stop. Foam is blowing on the lower portion of the back stop, asked rig hands to use some defoaming agent to lower the foam level. Operator is wash foam into pit at the time of inspection. Operator added defoaming agent to the blow pit at the time of inspection. North end of the pit is full of cuttings, operator began washing cuttings down to the south end at the time of inspection. Fluid level in the pits are good. Operator added extra pit liner to the pit, to prevent from dusting area. Top head drive is leaking 90 weight oil, drill stated that they are adding 1.5 gallons of oil every 4 hours. Cellar was full of 90 weight oil. There were approximately five empty five gallon 90 weight oil buckets in the trash. 90 weight will need cleaned up from the site. Called Ralph Gregory from Bass Energy and informed him of the leak and the oil around the surface pipe. Ralph called Ray the tool pusher for the rig, per Ralph and I are in agreement CapStar needs to shut down and make repairs and clean up oil.
