0079809309 2008-08-27 Drill / Deepen / Reopen No Violations Capstar rig 1 on site. Lease road is stable, operator constructed a rock apron at the entrance of the site, apron is approximately 80 in length. Operator was digging blow pit at the time of inspection, soil was clay, reminded operator to keep slack in the liner when they line pit. Operator was waiting on fresh water to start drilling CND. Operator will set one joint of CND cement to surface. Surface pipe was on site, pipe was a mixture of new and light used pipe, of different lengths. Drilled has measured and total pipe length.
0419684318 2010-06-16 Preliminary Restoration No Violations Well is a pump jack well. Operator has installed a barbed wire fence around the well to prevent livestock from coming into contact with the wellhead. Tank battery has one 210BBL tank in it, one 100BBL brine tank in it and a mechanical separator. Production numbers are on the 210BBL tank. PVC pipe drain was located in the north west corner of the dike. Pipe was open with no drain plugs. Contacted Dave Harcker in regards to the drain pipe. Pipe needs caps or removed. Identification sign was up. Lease road from the battery, down the hill to the wellhead had rill erosion forming in the tire ruts. Erosion will need to be monitored and repaired if it increases. Vegetation is established and affected area is stable.
0909911198 2017-12-14 Production Wells No Violations At the time of this inspection, the well was equipped with a pumpjack and the annulus and flow line valves were open. Production records for this well were current. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked and had legible signage. The lease access was stable. No violations observed.
1220542012 2008-09-04 Administrative Inspection No Violations Call from Richard tool pusher Capstar Rig #1. TD at 5:30am today. Loggers will be on site at 2:30pm. Show of gas in the Newburg and water in the Newburg. Operator put 10 bags of ready mix cement around the surface hole. Surface cement fell back after job was complete.

Cement appears to have fallen. Driller was sleeping, asked one of the hands to check to see if they can see how much the cement fell back when hooking up BOP. Add a bag of quick Crete if needed. Will have operator grout during long string if needed.
