The tank battery consists of a 210 BBL production tank, a 100 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The load lines are plugged and locked. The identification sign is located at the lease road entrance. The lease road is in stable condition.

1219945045 2008-08-28 Preliminary Restoration Violations Noted On site at a request of Geauga Soil and Water. Tank battery has been installed. Well is a pump jack well. Dozer on site, operator has been working on final grading. Area around the well head is ruff graded. There is sediment in the drainage ditches that flow offsite. The first 200 of the lease road is stable and constructed out of durable material. The remainder of the lease road to the well head has been graded, it is not constructed out of durable material and is not stable. Called Rennolds Resources, they stated that they have purchased straw and seed, they have not completed the work because of the rain the past day. I informed them that work should have been done 2 months ago, drainage needs to be controlled immediately. They have purchased extra straw bails to stake down to control drainage. They stated that they had straw bails installed, the strings had broke and the straw washed away. Requested reclamation be completed within the next week. Grading, mulching and seeding needs to be completed within the next week. Erosion and sedimentation needs to be controlled immediately.
1323725463 2008-09-17 Preliminary Restoration No Violations Majority of the site has been graded and seeded. Vegetation has began to germinate. Pit area has been ruff graded. Called company left message for David Rennolds that Im extending the NOV to 09/26/08 to complete grading and seeding. Operator has attempted to control sedimentation, four bails of straw have been placed before a intermittent stream south of the well. Lease road is stable.
1328691597 2012-07-09 Production Wells No Violations My inspection today found the following: A 5 gallon bucket full of crude oil at the wellhead. A 5 gallon bucket half full of crude oil at the tank battery and an empty plastic container that had oil in it, laying in the tank battery. Belt guards are off of the pumping unit. I contacted the operator and requested the site to be cleaned and the belt guards replaced.
1342450404 2012-07-16 Production Wells No Violations I was on site to today to follow up on work I request the operator complete on site. The operator removed used oil containers and five gallon bucket of crude oil from location. I five gallon bucket remains under the tank drip. This is allowed. Drip is used when unloading brine from the production tank. Belt guards are off of the pump unit and the compressor. It appears the equipment is under repair. Compressor is covered with a blue tarp and part of the pump unit has been dismantled. The lease road is stable. The identification sign is posted at the entrance of the site.
1603706988 2018-08-23 Production Wells No Violations I conducted an inspection on this well. At the time of this inspection, the well was equipped with a pumping unit and the valves on the tubing and flow line were open. Production records were current. The tank battery consisted of one 210 BBL tank, one 100 BBL tank, one 50 BBL tank and a mechanical separator. Valves on the separator were open. The tank valve on the 100 BBL tank was missing a plug. The tank battery was diked and had legible signage. Lease access was stable. I contacted Dave Rennolds with Rennolds Resources, LLC and left a message requesting a plug be added to the 100 BBL tank valve.
