The tank battery, separator, and wellhead are surrounded by a wooden fence. The tank battery consists of a 100 BBL production tank. The dike is in good repair. The load lines on the tanks and the man doors on the fences need locks installed. The heat treated separator has the proper spacing from the wellhead and the tank battery. The proper identification is posted on the tank battery fence. The lease road is in stable condition. I have contacted the company about correcting the issues.
1223047369 2008-10-03 Preliminary Restoration Violations Noted Entrance of the lease road, both sides of the road need graded, seeded and mulched to prevent erosion. Site needs final grading, seeded and mulched. Vegetation needs to be established to prevent erosion. Erosion damage behind well to the east needs controlled and repaired. The 210 tank is more than three feet above the highest portion of the fence. Tank battery is in the woods and not causing nay issues, a waiver will need to be submitted. Well ID sign is up. Well is a pump jack well. Well spacing is good. Site needs final grading, seeded and mulched. Drainage from site is causing erosion behind the well head. Stream bank is being washed away by the drainage. Drainage controls need installed to prevent further erosion damage. Erosion damage needs repaired

1531493624 2018-07-13 UrbanProduction Wells Violations Noted On 7/12/2018, I received a call from Marianne Hartman - DOGRM Emergency Response, with a complaint of a gas leak at this location. Mr. Carl Hill, landowner, called in the complaint to DOGRM. I contacted Mr. Hill about the leak and then proceeded to contact David Oil Company, LLC. I spoke with the operator at David Oil and discussed the leak at this well site. The operator stated she would contact the field supervisor to go out to the site and repair the leak.

I was on site on 7/13/2018, to conduct an inspection, regarding the gas leak. The leak at the well location was still present. The well was a plunger lift assembly and the gate valve on the tubing was closed. The 2 inch flow line valve off of the tubing was closed. There was a 2 inch line off of the well, opposite the 2 inch flow line that had a tapped bull plug and pressure gauge on it. The valve on this line was open and the gas leak was coming from the top of the pressure gauge. I contacted David Oil Company and spoke with the operator regarding the leak. She stated that she would call a well tender to come out and remedy the leak. The well was fenced and locked. The tank battery consisted of a 100 BBL tank and a mechanical separator. Valves on the separator were open. The separator was fenced with a locked door. The 100 BBL tank was diked and fenced. The fence did not have lock on the door. The signage was located at the 100 BBL tank fence but needed to be updated to include the new company name and contact information. Lease access was stable. While I was still on site, the well tender arrived and closed the valve on the leaking gauge. Well leak stopped. Compliance notice closed.
