0338959304 2017-01-11 UrbanProduction Wells No Violations During this inspection, it was observed that the well was equipped with a gas fired motor on the pumpjack and the valves on the wellhead were open. There was a domestic line with a meter, drip bottle and a regulator off of the 4.5 inch casing with valves open. There was standing water around the well casing and bubbling of that water was observed. The last reported production for this well was in 2015. The tank battery consisted of one 210 BBL production tank, one 100 BBL brine tank and a mechanical separator. The valves on the separator were open and the chart showed production. The tank valves were equipped with flat plugs. The tank battery was diked but the dike needed to be rebuilt along the south side of the tank battery. The signage was located at the tank battery and will need to be updated to reflect the new ownership information. The lease access was stable. I contacted Rick Osbourne owner of the David Oil Company (216-215-1313) and spoke with him regarding the leak at the well, rebuilding the south side of the dike and updating the signage.
1456410582 2016-02-25 UrbanProduction Wells No Violations My inspection found the well to be equipped with a pumpjack. There is a domestic supply plumbed into the 4.5 casing on the wellhead. The are two valves on the wellhead that need to have plugs installed in them. The tank battery consists of a 210 BBL production tank and a 100 BBL brine tank. The load line valves need locks installed. The south side of the dike around the tank battery needs to be rebuilt. The separator is outside of the tank battery and is 50 ft away from the well. The well is currently being produced and production records are up to date. The proper identification is located on the tank battery. The lease road is in stable condition. I have contacted the company to have the issues corrected.
