0212769072 2012-04-30 Production Wells No Violations Returned to location in the late afternoon. Rain was forecasted at night in the area. I constructed a dam using wood stakes and plywood south of the culvert to prevent residual brine from entering the culvert. I dug a narrow sump along the east side of the lease road to prevent residual fluid from flowing across the field.
0254529053 2012-06-01 Production Wells No Violations I was on site today to check the status of the site remediation after the brine spill. Vegetation is starting to germinate on re-soiled areas. The weather the past three weeks has been dry. Area is currently receiving rain, this should assist in germination of the grass seed.
1037609137 2012-05-10 Production Wells No Violations My inspection today was to check the status of the brine spill clean up. The area has received on 05/09/12. I checked standing water outside of the tank battery. I tested puddles of water in two locations, first location was east of the tank battery, direction of the brine flow, pooling water had 2400ppm Cl-. Water in the gravel north of the tank battery had 4000ppm Cl-. Total free fluid was approximately 20 gallons. Depth of water would not allow for removal. Remaining Cl- on location are diluting. The field that was scalded has been re-soiled and seeded.
1155232827 2012-09-24 Annular Disposal No Violations The unit consists of a 2-tank battery (TB) with mechanical separator with plunger-lift at wellhead. The spacing is in accordance with the rule, and the valving and mechanicals are in good repair. I did not observe any signs of leaking product or gas vapors. No evidence of anuular disposal. The tank battery dike is adequate. The unit ID is good. No violations noted.
1339431357 2012-05-07 Production Wells No Violations I was on site today to check the status of the brine release clean up. Site has been stabilized. Pasture has been re-soiled, seeded and mulched. Dike has been reconstructed.
1690670248 2013-09-23 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1722764837 2012-05-10 Production Wells No Violations On 5/10/2012, I performed a production well inspection status check with DNR Inspectors Tom Hill and Mike Dinsmore. We were inspecting the progress of a clean-up from a brine overflow. Refer to DNR Inspector Tom Hills inspection report.
1947463093 2012-04-30 Production Wells No Violations I was on site today at the request of the company. The 50 barrel brine tank in the battery leaked, releasing 2 to 5 barrels of brine. Drainage away from the tank battery is east and south. There was a brine scald east of the tank battery, in a horse pasture. The scald was 130 feet long and on average 15 feet wide. The brine escaped from under the tank battery dike. The brine also flowed south along the lease road, towards the wellhead. Scald was approximately 2 feet wide and 40 feet in length. The scalding stopped before a culvert on the east side of the lease road. The culvert catches drainage form the south and flows north into a pond. I tested the water at the discharge of the culvert, where it empties into the pond. The water tested at 357ppm chlorides. It was agreed with the operator that the following remedial actions would take place: remove 2 to 3 inches of soil from the pasture in the location of the scald and replace with clean topsoil. Bio-remediate lighter scalded areas. Pull tank battery dike out and rebuild using clay to prevent escape of fluid. Remove the brine tank. Operator will have crew on location in the AM of May, 1, 2012. It should be noted that the scald was noticed by the pumper Thursday, April 26 and not reported to Enervest Supervisors until Monday April 30, 2012.
1951534449 2012-05-01 Production Wells No Violations I was on site today to check the status of the brine spill clean up. Operator constructed a sump in the tank battery and flushed the stone floor, to remove any free oil and brine. The operator removed the east and north side of the dike. There was stone under the dike, which allowed the brine to seep from the dike. The operator had removed a portion of the fence from the horse pasture and was preparing to scrape the scalded soil and remove from location. A load of clay for reconstructing the dike was brought onto location. I pulled a water sample, as a courtesy for the landowner, which will be tested by the DMRM lab. The landowners water well is approximately 600 feet north west of the brine scald and the tank battery. Subsoil on location is clay. No water well records were located for 10668 Plank Road, the location of brine scald. Review of the offset water well logs, show wells developed in either in the shale at a depth of 30 feet to +/- 112 feet, or a sandstone (Sharon Conglomerate) , below the shale. The gas well records show the Berea sandstone top is 267 feet in the area. Water well records reviewed indicate +/- 30 feet of clay above the shale in the area. This clay will prevent brine from moving into the aquifer.
