Surface erosion is 8 wide, 100 in length and 1 or less in depth and has stabilized on what remains of the sandstone base. Wetland crossing has been repaired. Well location has been reclaimed and the tank battery has been removed. Above ground piping remains at the meter run tap. Contacted company to enquired about the flow line. Was flow line removed or flushed and left in place? If it was not removed the Division needs a land owner waiver.

1286378940 2010-09-17 Plug / Plug Back No Violations On site for plug job. Clinton plug had been set the day before. Clinton plug was tagged at 3419.. Ripped 4.5 inch pipe at 2789. Pulled 8 joints, 338 feet of pipe. Broke circulation ran cement, set lime plug at 2458 to 2158. Pipe is JSS Newport Steel, 10.5. Plug #3 pulled 48 joints, broke circulation, ran cement plug #3 set at 435 to 235. Pulled up 5 joints, broke circulation with cement, ran cement till it returned to surface. Pulled last joints topped of with cement. Cement held at surface.

1871384295 2010-09-14 Plug / Plug Back No Violations Inspected site with Pluggers Inc. Operator will need to cross wetland. Suggested operator place geotextile fabric down then place stone down to cross soft area. Rock and fabric would be easily removed during reclamation and minimize damage to area. Vegetation in the area is saplings and new growth willows.
