0559960747 2013-09-17 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The well and tank battery are in good repair. The tank battery is surrounded by a chain link fence. The tank battery consists of a 100 BBL production tank and a mechanical separator. The proper identification is located on the tank battery. The lease road is in stable condition.
1535030463 2018-08-23 Production Wells Violations Noted I conducted an inspection on this well site in response to a call from Emergency Operations Manager, Trish Cartmell. Ms. Cartmell explained that a call was received on 8/22/18, regarding a gas leak on the property containing this well. Fred Romeo, Emergency Operations Coordinator responded to the call on 8/22/18 and met with Bill Fitzmartin from M&R Investments on site. Mr. Romeo confirmed that a gas leak was present at this location. On 8/23/18, I conducted an inspection on this well location. The well was equipped with a plunger lift assembly and not in production. There was a domestic line plumbed off of the 4.5 inch casing which was reduced to a 1 inch line and equipped with a regulator. The valve on the domestic line was open. The tank battery consisted of one 100 BBL production tank and one mechanical separator. Valves on the separator were open. The tank battery was diked and fenced with legible signage. The lease access was stable. I contacted Bill Fitzmartin from M & R Investments inquiring about the gas leak. Mr. Fitzmartin stated that the flow line between the well and tank battery had developed a leak. On 8/22/18, M&R Investments had responded to the leak, and isolated the location of the leak by shutting in the well and relieving line pressure to stop the leak. At the time of this inspection, no gas leak was observed. Mr. Fitzmartin also stated that the flow line still needs to be repaired.
