0013581111 2013-03-19 Production Wells No Violations The well is equipped with a concoil wellhead and is surrounded by a chain link fence. The tank battery consists of a 210BBL production tank, a 35 BBL brine tank, and a mechanical separator. The well and tank battery were in good repair. There was no identification sign on location. I left a message for the well tender to put proper identification on tank battery. The lease road was in stable condition.

1613887651 2018-07-05 UrbanProduction Wells No Violations During this inspection, the well was equipped with a Concoyle plunger lift assembly. The valve on the wellhead and 2 inch flow line was open. The well was fenced. Production records for this well were current. The tank battery consisted of one 210 BBL production tank, one 50 BBL poly brine tank and a vertical mechanical separator. The valves on the separator were open. The top of the 50 BBL brine tank was crushed, and has been disconnected from the other production equipment. Fresh water was inside the 50 BBL tank. The tank battery was diked and had the proper, legible signage. The lease access was stable. No violations observed.
2009773319 2016-11-18 UrbanProduction Wells No Violations I conducted an inspection on this well. The well is equipped with a Concoil plunger lift assembly and the valve on the wellhead was open. The well was fenced but needs a lock on the man door. The last production records for this well were submitted in 2014. The tank battery consisted of one 210 BBL production tank, one 50 BBL poly brine tank and a heater treater separator. The separator had the proper spacing from the production tank. At the time of this inspection the poly brine tank was empty but the top of the tank was crushed. The valves on the separator were open and the chart showed production. The tank valves were plugged. The tank battery was diked and had the proper, legible signage. The lease access was stable. I contacted Alan Lorinchack 440-773-4101 from Summit Petroleum about installing a lock at the well location and replacing the crushed brine tank.
