0786204293 2019-04-22 Plug / Plug Back No Violations MH Oil Field Services on site to plug the open portion of the well to surface. MH opened the 2 inch valve on the wellhead, no gas or fluid observed. MH removed the wellhead. Cement mix water was pumped out of the stream. Mix water had a PH of 7. Low range Quantab showed no CL in mix water. MH batch mixed 13 sacks of cement (Saylors 94 lbs. /sack) and filled by hand 13 sacks to surface. Casing was cut off at 30 inches below grade and a monument plate with ODNR and 60108 was installed on casing. Excavated area was filled in and site graded. Grass seed and straw were applied to affected areas.
2123821226 2016-09-22 Orphan No Violations Orphan well status inspection, accompanied by DOGRM Geologist Jake Glascock: Upon inspection, it is observed that this well is idle, incapable of production, and is likely orphan. The well consists of 7 diameter surface casing with a 7-2 diameter swage and a reducer which elbows into a .5 diameter fitting with a gauge. The gauge does not register any pressure. There are several holes in the 7 diameter casing just above ground level. No gas, oil, or brine is observed to be leaking from the casing. There are no lines attached to the well and there is no other production equipment associated with it. There is a water well which is no longer in use located 55 south of the well. The residence receives drinking water from Gallia Rural Water. Overhead power lines for the residence are present near the well location. There is a buried water supply line as well as buried satellite/internet lines near the well location. Access to the well is off of County Road 115. A recently rebuilt concrete bridge has to be crossed to access the property. The property has changed ownership since the initial complaint and is now owned by Mr. James Ditty. Mr. Ditty has been given a right of entry form which he states he will sign and return.
