I explained that we had a meeting with myself, Robert Stonerock, Tom Tomastik, and Steve Opritza. To review the plug plan submitted by Continental. I reviewed the Divisions Concerns about the perf and squeeze. I also explained our request for a segmented bond log. Ed was not satisfied with our discussion. I suggested that he talk to someone else from the meeting. I referred him to Tom Tomastik and Steve Opritza. Ed has requested specifications on the vault and vent system.

They did not use a mud anchor on the tubing. Circulate gel and flake to surface. Pump cement plug from 3221 to 2700. WOC.

Opened needle valve could hear and see gas (low pressure and continuous volume) Closed and opened needle valve multiple times each time pressure increased slightly then flowed at constant rate. Pressure is low around 35psi.

I told Merl that the well is still leaking gas around the outside of the 7 casing. I asked to set up a meeting so we could discuss cement procedures and I requested all records for the well that pertain to the drilling and cement jobs for this well in addition to a detailed list of the casing and any equipment that was used down hole.

Cement stayed at surface.
0653513675 2008-08-11 Plug / Plug Back No Violations Well was plugged back on 8-10-2008. From 1850 to 1600. This is a kick off plug for drilling horizontally. After logging it was determined that a possible fault was located between 1600 and 1500. 130 sacs of class A cement were used. The cement did not set up and is causing problems with the drilling of the horizontal curve.
0681497094 2009-01-14 Completion Testing No Violations Gas and brine are bubbling around the wellhead. Appears to be coming from the backside of the 7 casing. Similar to the Corbin well. I took a video with my digital camera. I tested the water with a high end Quantab. The tab measured 7.8 on the strip which is 4907 ppm cl. The temperature was 20 F and every thing on location was frozen except at the well head where the brine was bubbling steadily.

1227285075 2009-06-29 Plug / Plug Back No Violations Arrived at 8:30 am to check the status of plugging activity. It was noted that the immediate area around the well bore has been excavated. The Company Representative, Mr. Greg Lawson states 40 lbs on 7 casing. Observed soap, in bottle, sprayed outside the 7 casing. No gas was observed behind the 7 at the surface. Due to a meeting that had been organized to discuss the plugging of this well, Mr. Tom Tomastik was advised of the well status and proposed plugs by telephone. The plugs were approved. It was also recommended to leave the backside open and to not shut-in when plugging. This information along with the plug intervals were provided to Mr. Lawson whom, agreed with the proposed plans.

1323257430 2009-05-07 Preliminary Restoration Violations Noted Spud date 8-2-08, Restoration due 5-2-09. Site is rutted with standing water. No attempt to vegetate site. Site is bare ground. No identification on well. Well is equipped with tubing. Robert Stonerock (South Region Supervisor) accompanied me on the inspection. Well is leaking gas and fluid around the backside of the 7 inch casing. Gas is bubbling like a rolling boil in a 6 foot pool of brine at the wellhead. I called Merl Richards (company representative) at 6:18 pm on 5-7-09 and informed him of the violations.

Pumped cement plug from 3605 to 3200 took sample to determine when safe to tag TOC plug. Had trouble could not clear tubing after running back to tag 1st plug. Pulled all tubing and had cement inside 35 joints of tubing.
1588809931 2009-09-22 Plug / Plug Back No Violations TD 711. Cast Iron Bridge Plug set at 695 Pressure tested to 586 psi for 15 minutes. At 15 minutes pressure was 563psi. Tubing ran to 688 Circulate gel to surface. Pump cement 17 bbls Class A 15.6 lb (81 sacks). Plug set from 688 to 275. Pulled tubing to 275 and pumped and circulated cement to surface. Cement stayed at surface.

1782969900 2009-10-06 Plug / Plug Back No Violations Mix 1 bag Portland Cemex type 1 (94lbs) with 1.5 bags Gypsum ( Plaster of Paris and Portland Cement). Poured mixture into inverted 4.5 by 2 swedge ( used as a funnel) that was connected to a 2 inch valve. Removed swedge and installed a 2 inch bushing with a test rig attached onto the valve. 200 psi of Nitrogen applied through the 2 inch valve. Small leaks were observed on the top welds of a fabricated 7 inch by 2 inch swedge. Repairs were attempted to seal the leaks. Maintained 200 psi Nitrogen for 15 minutes. Increased pressure to 300psi shut 2 inch valve and removed test rig. Leave valve closed a minimum of 24 hours.

1913585495 2008-12-19 Completion Testing No Violations A pool of fluid is surrounding the wellhead approx 6 feet in diameter. There is a constant bubbling like a rolling boil that is coming up beside the casing. I was able to extend a small bucket and capture some of the fluid bubbling at the casing. An high range Quantab was used. It measured 8 on the strip that is 5408 ppm Cl. I took a video of the leak. The drill pit has been filled in. I used my gas detector and could not detect any explosive levels near the well. The wind was blowing and could have dispersed the gas. I have informed Merl Richards of this problem and he is working on getting the records to me so that we can determine what is causing the leak.
