0125345496 2018-11-21 Drill / Deepen / Reopen No Violations Baker Hughes on site running neutron, gamma ray and resistivity logs. The well reached a total depth of 3799 feet. I met with Mr. Don Huck with Artex Oil Company and stated they would be running 4.5 inch diameter production casing. Artex Oil Company has a power broom on site and is keeping the roads clean of mud and debris.

0584172678 2018-12-04 Completion Testing No Violations My inspection revealed the well is being tested and completed. There was a service rig on site during my inspection. The public road was free of mud and debris.
1144494778 2019-01-18 Plug / Plug Back No Violations Formation Cementing arrived on site at 8:00 AM. They loaded the hole with fresh water and circulated the hole to be sure they would have circulation. They mixed 30 sacks of class A cement and added 40 pounds of LCM (cotton seed hulls) and 50 pounds of accelerator to ensure the cement had a fast set time. Bishop Well Service removed the tubing that was used to set the bottom hole plug. The remainder of the plugs will be set at a later date.
1273059868 2018-11-14 Drill / Deepen / Reopen No Violations Kilbarger rig number 5 reached a total depth of 646 feet. They ran 605 feet of 20 pound per foot 7 inch diameter surface casing. Formation Cementing used 100 sacks of class A neat cement to bring the cement back to surface. Approximately 1 barrel of cement was returned back to surface.
1376317603 2019-04-18 Final Restoration No Violations My inspection revealed the final restoration has not yet been completed. I met with Kilbarger Construction and they said after the Kuhn Harvey well is finished, they will be working on the final restoration for this well.

1548273967 2019-01-23 Plug / Plug Back No Violations Bishop Well service was on location with a derrick service rig. The well was drilled to a total depth of 3799 feet and was equipped with 9.625-inch diameter conductor to 372 feet cemented to surface, 7-inch diameter surface casing to 604 feet cemented to surface. ODNR inspector Kip Thompson witnessed the bottom plug. During my inspection today, I witnessed Bishop rip the 4.5-inch diameter production casing at 2500 feet. 15 joints of 4.5-inch diameter casing were pulled leaving the bottom of the casing at 1866 feet. Formation Cementing arrived on location and pumped a Clinton plug within the 6.25-inch diameter borehole from 1866 feet to 1666 feet. The plug consisted of 40 sacks of class A cement, 2 percent calcium chloride, and lost circulation material. Bishop tripped out 15 joints of 4.5-inch diameter casing leaving the bottom of the casing at 1210 feet. Formation Cementing pumped a Big Lime plug within the 6.25-inch diameter borehole from 1210 feet to 1010 feet. The plug consisted of 40 sacks of class A cement, 2 percent calcium chloride, and lost circulation material. Bishop tripped the 4.5-inch diameter casing out of hole and the 7-inch diameter surface casing was shut in. Weather today was 40 degrees with consistent rain.
1619141695 2019-02-22 Final Restoration No Violations My inspection revealed this well is plugged. The well cannot be passed for final restoration at this time. The site needs graded. The site will be graded when the soil drys enough to allow the site to be graded. No seed or mulch will be required as this is a agricultural field.
1691891234 2018-12-07 Completion Testing No Violations My inspection revealed Bishop Well Service has completed the perforations and has acidized the well. They have buried a production line and connected the line to the existing tank battery. The public road was free of mud and debris. Bishop Well Service will move there service rig to 3404521505 to plug this well.
2104726420 2019-05-15 Final Restoration No Violations My inspection showed Bandit Energy on site working on finishing the final restoration. They were working on finishing solidifying the last pit. When this is done, the top soil will need to be respread as this well was located in a agricultural field. The well does not yet meet requirement for final restoration.
2122365873 2019-01-24 Plug / Plug Back No Violations Bishop well service ran tubing down to 703 feet. Formation Cementing set a 40 sack plug that contained 2% calcium chloride. This plug was the bottom of surface plug. They tripped tubing out to 200 feet below top of the surface casing and Formation Cementing ran 40 sacks of cement with 2% calcium chloride. This plug came back to surface inside the surface casing. The surface casing will be cut off 30 inch below grade and final restoration will be completed as weather allows.
