0027575999 2010-11-16 Production Wells No Violations On 11/12/2010 at 9:57 a.m., the Division received a landowner complaint through the Ohio EPA alleging flowline leak in Hocking County. The Ohio EPA will be contacting the well owner. On 11/12/2010, the Division contacted the landowner to discuss the complaint. The landowner states that a steel flowline on top of ground, in the Kessler field, located approx 100 from the well squirts oil when pumped. The line was clipped by an individual named Sweeney 5 weeks ago. They have called the company but no repair has been done and the pumper continues to pump well. An inspection conducted with Dan Goins revealed that the subject well and line are located in Fairfield County and routed to a common (shared) tank battery in Hocking County. Note: Two additional Bernard Schmelzer wells owned by Northwood Energy are located in Hocking County. Approximately 20 of the subject line has been replaced with no observed contamination and pollution surrounding the line. Met with the landowner and discussed the inspection results. A GPS reading was obtained from this inspection. No violations were noted.
0389696300 2014-11-26 Production Wells No Violations The tank battery has been removed and the site has been restored. This was done because the tank battery was within township road right of way. The well is not plugged but Northwood plans on plugging the well. The request to move the tank battery came from township trustee Scott kitsmiller.
0594372488 2016-05-05 Final Restoration No Violations The purpose of this inspection is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well existing. The site was restored. Location has met 1509.072 B requirements.

0896150434 2014-10-01 Production Wells No Violations I met with Jim Kilbarger with Logan Gas. We went over the meter and piping that need removed out of the R/W of the township road. Jim agreed to move the equipment within two weeks.
0990255705 2014-07-31 Production Wells No Violations I received a request from Hocking County regional planning employee Audie Wykle. Audie received a call from Marion township trustee Scott Kits miller. They informed me their was a tank battery and gas meter in the township road right of way. I went to the Hocking County courthouse and received aerial photographs of the area. The road is Schmeltzer road and is a township road that is not maintained. I told Trustee Scott Kitsmiller that I could not issue any compliance notices due to the fact I am not able to determine where the centerline of the road is. Therefor I can not determine if the tank battery is in spacing violations. I have asked to contacted when the road is located and then I can determine if the tank battery is in violation.
1421265494 2015-01-14 Plug / Plug Back No Violations R&B Drilling on site, they set the bottom plug with 30 sacks of class A cement on 01/13/15. They ripped on the 4.5 casing with out success. Appalachian Well Service on site to shoot of the 4.5 casing at 1908. The remainder of the plugs will be set on 01/15/15.
1421350654 2015-01-15 Plug / Plug Back No Violations R&B Drilling on site. The lime plug, Berea plug, bottom of surface and surface plug set today. The plug did not circulated. Corp Oil Field is removing flow line from the site. The surface plug fell back to 150. I instructed Northwood Energy to grout the surface plug after the cement set up.
1469498087 2015-02-02 Plug / Plug Back No Violations The tank battery has been removed, the gas meter that was in the township road right of way has been removed. All that remains within the township road rite of way is a steel gas line and a vertical drip. I will contact Logan Gas to find out why they did not remove the drip and the line.
1545196524 2014-09-30 Production Wells No Violations I received a request for a inspection from Township Trustee Scott Kitsmiller. Scott had Hocking county engineer to stake the centerline of Schmeltzer road. This road is a unmaintained road. The landowner plans on bring the road back to Township standards. The issue with this the tank battery is in spacing violations and will need to be moved. Northwood Energy has applied and received a plugging permit for this well.
1962842022 2014-08-01 Production Wells Violations Noted Production well inspection. One of the 100 barrel tanks is seeping oil. I contacted Mike Corp to have a vacuum truck remove any fluids from both 100 barrel tanks. The truck arrived on site at 10:00 AM. Both tanks were vacuumed out and the small amount of water was removed from the dike.
2033050980 2016-06-09 Plug / Plug Back No Violations I received a call from Robert Scmtlzer. He said the site where the well as located was not restored. To small erosion ditches remain. I contacted North Wood Energy to have the site graded and seeded.
