0101614473 2010-05-24 Production Wells No Violations FOLLOW UP TO NOV # -799061400 I inspected this well location, and well number 34-045-2-0349 today at the request of landowner Don McCray. He indicated that these two wells have not produced for several years and that the pumping units were removed from them a couple years ago. Then tank battery was also removed. The well locations are overgrown with vegetation and the wells are not producing. There are rods and tubing in both wells. There is no form of identification posted at either of these two wells. By July 27, 2010 the well owner will put this well back into production and post proper identification or will obtain a valid plugging permit and plug the well.

No production records have been filed since 1993. VIOLATION OF 1509.11 O.R.C. OWNER NEEDS TO FILE PRODUCTION RECORDS PLACE WELL INTO PRODUCTION AND / OR PLUG SAID WELL. If the well is placed into production ID needs to be placed at the well head. I called Mr. Nutter and advised him to file his production report with Columbus. 1556196296 2011-07-13 Final Restoration No Violations The Division received a landowner complaint alleging that a plugged and abandoned o/g well is leaking crude oil. An inspection conducted with Mineral Resource Inspector, Andy Thomas and the complainant revealed no signs of leaking oil and gas well. The area allegedly containing crude oil was actually iron scales/bacteria. The location was along the lease road and below well location. High Range Quantabs were also placed in the pooled fluid and showed no Cl-. A freshwater spring exists above the well location therefore, hydric soils exist on this location. Note: The complainant states pipe from spring had been damaged by plugging contractor and was replaced. The complaint will be closed. Discussed the Final Restoration with the complainant whom, states that he is satisfied with the work performed by the well owner. An inspection of the well site revealed that the Final Restoration meets ORC 1509.072.
