1375893047 2013-08-07 Orphan No Violations This well is in the Orphan Well Program (Case #2004 08 038). Requested by Gene Chini to inspect well for current conditions and collect new photos. Well is drilled into the Sylvania Sandstone and is a known hydrogen sulfide source in this region. A precast concrete vault has been placed over the well and is overgrown with vegetation. A metal fence post has been placed along the edge of the field to mark the location of the well. Access to the well is through the top of the vault. The well was covered with a plastic garbage bag and a coffee cup. The seven inch surface casing is visible at grade. The 4.5 inch production casing sticks up about 12 inches with a collar and a swage to a two inch valve. The collar has been removed and a piece of visqueen has been placed over the well and then the collar with the swage has been place back onto the well. This was not observed in the photos from 2004. The well head may have deteriorated and fallen off, then replaced by the property owners. Gas readings were collected at the well head and inside the vault. The LEL had readings of 1.0 %.
1403184675 2014-06-18 Orphan No Violations Inspect well for condition. Utilize gas meter to evaluate well. Gas Meter indicated 0.10 ppm Methane; 3% Lower Explosive Limit; 1 ppm Carbon Monoxide; and zero for Hydrogen sulfide. Reading were collected at well head inside the protective concrete vault. A Pole barn structure has been constructed on the property. Spoke with current landowners the Buyers regarding the well.
