At the time of this inspection, I noted the following: a wellhead with flow-line valve and supply connections which were open. The claimant of the well, Peri Drosos, has previously indicated that the well had been in active use for Mr. Drosos residence. Mr. Drosos has indicated that he sells gas to the residents of the house located on the property on which the well is located. Mr. Drosos has not been responsive to attempts to direct him to establish proper registration. I contacted Mr. Drosos on the date of this inspection and he indicated that he has not made any progress towards establishing registration. Im having new registration paperwork sent. |

Reportedly, construction activity occurring at 16908 Edgewater was feared to have caused damage to subsurface gas lines that supply gas to the house at 16908. The landowner (Mr. Pericles Drosos) reports that the well is located at 16905 Edgewater. Mr. Drosos reports that gas is currently being sold to other homes. Mr. Drosos claims to be the owner/operator of the well. The damaged service line was repaired prior to my inspection. The well is located in the back portion of the house located at 16905 Edgewater Dr. I observed the wellhead to be located in a subsurface vault structure with a cast iron, round access lid. The wellhead is approximately 30 inches below grade and consists of a diamond shaped 2-bolt cap with 1.5-inch service line connection. There is free oil ponded within the vault which will need to be cleaned out to make a thorough evaluation. At the time of this inspection the well is not registered. I advised Mr. Drosos about the registration, bonding, and insurance requirement and that I will be arranging for the documents to be sent to him. |

At the time of initial discovery, the reported owner was advised about requirements for registration, bonding, and insurance. At the time of this inspection, proper registration, bonding, and required insurance have not been established. The owner must provide registration, bonding, and insurance documents or be subject to enforcement action by December 23, 2015. |

During this inspection I met with the owner of the house where the well is located. The current owner of the house, purchased the house within the last year. They have an informal agreement with the claimed owner of the well (Peri Drosos) to receive gas at a discounted rate. They wanted to make sure everything was properly maintained. At the time of this inspection, I inspected the well and associated vault. There has been some oil accumulation in the vault over time, indicating a possible leaking fitting. Subsequent to this inspection, I spoke with Peri Drosos regarding on-going registration efforts. Peri has indicated that he was negotiating with RA Gibson Energy to take over well ownership, These negotiations were on-going. |

I, along with District Supervisor Ochs, inspected the well location and surrounding area and found the well condition to be stable with no problems evident. I contacted the claimed owner Peri Drosos regarding progress on potential transfer of the well to a new owner. Peri reports that he continues to be uncertain whether he wants to take ownership or transfer the well. I advised him that the Division was working on proceeding with enforcement since he had not responded to the previous directives to properly register, post bond and insurance as required. |
| 1555081680 | 2019-04-05 | Production Wells | Violations Noted | I spoke to the landowner recently where the well is located, he indicated that a business partner of Mr. Drosos had dropped off a gas detector for his house. The landowner also indicated that there are intermittent increases in gas odor coming from the well. The odor increases on calm days. I also recently spoke to the Lakewood City Fire Marshall, he expressed serious concerns that the well will be properly operated and maintained. He also indicated the well leaks oil into the storm sewer. It has been reported that the vault the well is in, has a drain that discharges into the storm sewers.
Was on location with Fred Romeo DOGRM Emergency Response Manager to perform a gas leak detection survey with a FLIR Thermal Camera. The well is in a vault southwest of the corner of the residence owned by Mr. Hudson. The vault entry is covered with a steel utility access hole cover. When the utility access hole cover was lifted, FLIR Survey indicated natural gas venting from the vault. The vault is vented underground to the northeast, the vent line comes out of the ground adjacent to an oak tree, and then runs up the tree. FLIR Survey showed gas venting from the vent stack. The escape and venting of gas indicates that the well doesnt have integrity. In addition, previous inspections indicated escape of crude oil into the vault. The following compliance issues shall be addressed: The owner of the well shall bond the well, obtain insurance and register with the State of Ohio DOGRM. The owner shall within 7 days submit a plan to the DOGRM to repair or plug the well. Free Crude oil and water shall be removed from the vault. The well shall be repaired or plugged by 05/12/2019. |
