0691784729 2012-10-05 Orphan No Violations On 10/5/12 returned to well site to cement bottom plug from TD to 450. Set plug using 133 Sacks Class A cement with no additives. Water weight of cement was 8.9 pounds per gallon and water temperature was 60 degrees with a ph of 7.5. Plug down at 11:00 AM. See attached cement tickets.
0857013120 2012-10-15 Plug / Plug Back No Violations On 10/15/12 I visited the well site to check on well restoration. The area around the well was swept with a magnet and all debris, drilling equipment and rental items were removed. The wellhead area was covered with dirt approximately 1 deep and plugging operation should be considered complete.
1348500349 2012-09-25 Orphan No Violations On 9/25/12 I returned to the well site on this date for the continuation of drilling and plugging of this well. Upon arriving on the site it was discovered that approximately 5 Barrels of oil had leaked from the well overnight. R.A. Gibson Associates began cleaning up the site by pumping excess oil into the 1/2 tank on site and cleaning up the residue using PeatZorb and diapers. It was decided to set a 40 x 10 drive pipe around the original borehole to alleviate this situation. After drive pipe was set, drilling of an 8 hole commenced at 10:00 AM to an approximate total depth (TD) of 558.
1349182528 2012-10-02 Orphan No Violations My inspection today was to continue witnessing of the drill and plug process. On site at 8:00 AM. Drilling is continuing at approximately 330. Picking up long strips of wood in drilling/circulating. Upon completion of drill through blockages, will determine plug footages.
1350498995 2012-10-04 Orphan No Violations On 10/04/12 I visited well to monitor continuation of drilling. Cleaned borehole to 863 at approximately 10:00 AM. Began circulating on well at approximately 11:45 AM @1,5 Barrels per minute until approximately 1:00 PM. Tripped out drill bit and ran 860 of 2 3/8 tubing. to prepare for cementing tomorrow, 10/05/2012. Estimate 133 Sacks Class A cement for 450 plug from 863 to 413.
1359483522 2012-09-24 Plug / Plug Back No Violations On 9/24/12 I visited this well to observe the completion of site set up in preparation for plugging of an oil well on this property. The surface of the area throughout the well site is asphalt and cement, no construction on site is necessary, The site is equipped with a 1/2 40 Barrel (BBL) FRAC tank and a 300 BBL overflow storage tank. The site is surrounded by chain link fence and no pits will be dug on the site. The wellhead area is approximately 6 x 6 square and is dirt. Drilling commenced at approximately 9:00 AM. At approximately 2:00 PM while making a joint connection at approximately 30 oil began to overflow the top of the joint. A shut off valve was installed and R.A. Gibson associates began remedial action to clean up the spill, using PeatZorb and diapers to soak up the oil. estimated less than 5 Gallons of product). It was determined that the oil was coming up from outside the casing in the ground.
1454447752 2012-10-03 Orphan No Violations On 10/3/12 I was at well to continue operation of drilling. Well is at approximately 496. Ran ten 31 joints to a depth of 863. The open 1/2 tank on ground has 70 Barrels of Sludge and 70 Barrels of oil in it. The 300 Barrel storage tank has 50 Barrels of freshwater and 80 Barrels of oil in it. Moving freshwater from storage tank to 1/2 tank and oil from 1/2 tank to storage tank.
