0093705418 2012-10-12 Orphan No Violations On 10/12/12, drilling on this well resumed at 9:30 AM, at a rate of 10 per hour. Drilling continued until 3:45 PM at a depth of 85. Because of the slow drilling conditions and the fact we had a solid base to set a plug , the decision was made to cease drilling and grout the well. A pressure head was configured with a gauge and 35 PSI of air was applied to the well, and the well was shut in for the weekend. The plan is to cement well using 9 sack grout on 10/15/2012. Operations for the day ceased at 5:30 PM.
0891517923 2013-07-22 Final Restoration No Violations Final restoration passed. No signs of well or equipment were found at well site. The well site is located in the backyard of house (4267 Root Rd).
1062527916 2012-10-16 Orphan No Violations On 10/16/12, my inspection showed that the well was holding 10 PSI. All equipment was removed from the well and there was no gas leakage detected. The casing was cut 30 below grade and a cap was welded on as required. The hole was then backfilled and the area graded to original contour and seeded.
1338478012 2012-05-30 Orphan No Violations I inspected this well located at 4267 Root Road. I was on site from 8:00 am to 4:00 pm. Contractor Ron Gibson was on site to drill out old well located behind the house. The contractor rented a Power Swivel from Timco and will be using a 4 tri-cone bit, on 2 7/8 tubing, to drill out the obstruction in the 10 casing (note visible wood inside the 10). They drilled approximately 58, with cuttings consisting of wood and shale but no oil. They tripped the tubing and bit out, shut in the casing and ceased operations for the day.

1351173673 2012-10-11 Orphan No Violations On 10/11/2012, Gibson and Associates began drilling the 7.875 hole through the top cement plug. Drilling began at 9:30 AM at a rate of approximately 10 per hour. The rate dropped to 6 per hour at 12:00 noon and at 3:00 PM drilling slowed to 5 per hour. While in the process of shutting down for the day at approximately 7:00 PM the drill string began to come out of the hole. It is believed that the hole collapsed below the drill string and the increased down hole pressure began to force it out of the hole. The situation was controlled and Gibson was able to remove a portion of the drill string from the hole, with the remainder left to hang in the hole overnight. Operations for the day were halted at 8:30 PM.
1373283920 2013-07-03 Final Restoration No Violations The area of the former well has been graded and re-seeded. The seeding has good established growth.
1493427073 2012-10-15 Orphan No Violations on 10/15/12, the well was bailed dry and a 9 Sack Grout mix was used to fill the hole from 85 to surface. A well cap, with pressure gauge, was installed and 35 PSI of air pressure was applied to well. I will return 10/16/12 to check pressure on the well.
1633070771 2012-06-01 Orphan No Violations I was on-site 8:00 AM to wittness plugging. The tubing was set at 830. They mixed and pumped 5 sacks of gel, then mixed and displaced a 300 cement plug from 830-530, using 83 sacks of Class A cement with 1 sack of Calcium Chloride and 1 sack of cotton seed hulls. Mixed on the fly and displaced cement. Pulled tubing to 350, mixed and displaced cement to 100. Shut in gas tight.
