0486146228 2016-10-05 UrbanProduction Wells No Violations I inspected this well location to assess the current status and condition. At the time of this inspection, I observed the following: The well is comprised of a 3-inch diameter casing riser with a series of connections, fittings, and drip tanks. The wellhead was located on the Lake Erie Cliff face with a wood and steel platform structure erected to allow access. A metal open staircase afforded access to the platform level which was approximately 20 feet below surface grade. Supply-line fittings connections continued to the surface grade area where a metering run was located. I contacted the reported well owner Ted Kuntz and advised that this well, along with the Kundtz 2 (#6-1058) and Kundtz 3 (#6-1059) wells lack proper registration, bond, and insurance. I sent the required documents to Mr. Kundtz and he advised that he would submit the registration.
1265525481 2016-11-23 UrbanProduction Wells No Violations I inspected this well as part of an effort to identify a responsible owner and establish appropriate registration, insurance and bonding. At the time of this inspection, the well is located along a in the back yard of a single-family home that is backed by Lake Erie. The visible well components consisted of a 3-inch steel riser casing with a series of connections supply-line and drip connections and fittings. The well and aforementioned fittings were located at the Lake Erie escarpment face, with a metal stair access to the wellhead. I did not record any meter-detectable gas readings in the ambient air near the well or the supply connections located at the top of the escarpment. I met with Ron Gibson of RA Gibson and Assocs., at the location and discussed the need for the owner to properly register, post bond, and report production as required under Ohio Revised Code (ORC) 1509.07 and ORC 1509.11. I also contacted Ted Kundtz regarding registration. I advised that I was sending the required paperwork for registration.
