0179232200 2016-05-06 Final Restoration No Violations I inspected this former well location as a follow-up to previously executed plugging and abandonment. At the time of this inspection, the former well location has been paved-over with asphaltic-style pavement, as part of site re-development for use as a Bob Evans Restaurant. The former well location is in the back parking area. No visible signs or evidence of the former well location remains. No further activity is needed. Final restoration is complete.
1365172589 2013-04-04 Plug / Plug Back No Violations I visited the site as preparations are underway to plug and abandon the well. Currently, the plugging contractor (Ron Gibson) is running 1.5-inch tubing into the existing 3-inch casing. The well is not currently producing any gas. Prior to my arrival at the site, I met with Fire Marshal Tim Dunphy to discuss the plugging activity and potential for production of fugitive vapors from the well and also from any oil recovered during plugging operations. The wellhead currently consists of 8 5/8 casing with a swage down to a 5, and then to a 3 casing. The wellhead had formerly been enclosed in a chain link fence with bollards at the corners. The enclosure features have been dismantled by Gibson. There are no other surface appurtenances associated with the well. Placement of cement plugs will commence tomorrow (4/5/2013).

2039116246 2013-04-05 Plug / Plug Back No Violations The cementing contractor (Formation Cementing) is setting up to place bottom plug through 1.5 tubing. Prior to running the plug, the hole is cleaned out, with some oil in the returns. 350 Class A cement plug is placed at the bottom of the 3 casing.
