At the time of this inspection, the wellhead was shut-in. I noted a detectable leakage of gas from the wellhead. I contacted Chris Mitchel, who claims ownership, and had not received a response at the time of this inspection.

The well is comprised of a valve tree with flow-line appurtenances. It is not apparent at this inspection the destination of the existing flow-lines. Im conducting additional investigation to attempt to identify the owner.
1179829088 2015-02-11 UrbanProduction Wells Violations Noted The well is comprised of a valve tree with flow-line appurtenances and located in the back yard of the referenced. There is no identification at the wellhead. At the time of this inspection, the valve connections are secure with no signs of leakage. Available information indicates the well is being utilized for heat for one or more rental housing units. At the time of this inspection, the property owner is not a registered owner and is not in compliance with the insurance and bonding requirements of Ohio Revised Code (ORC) 1509.07, and is not in compliance with the requirement to submit a Statement of Annual Production in accordance with ORC 1509.11. The owner must remedy these deficiencies by March 31, 2015 or be subject to additional enforcement action.
1410291311 2014-12-15 UrbanProduction Wells No Violations I inspected this well location which was brought to my attention by the Lakewood Fire Chief during unrelated activity, The well is located in the back yard of the duplex house located at 1208/1210 Cranford. There is a wellhead with 5-inch casing to reduction swage and 1-inch domestic supply. The duplex house at the property is currently unoccupied. It is unclear whether the well is connected to this dwelling or to other end-users. Additional research is underway to identify the landowner.
1517935103 2018-01-25 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous compliance notice issued for missing registration and associated insurance, bonding, and required production reporting. At the time of this inspection, I noted the following: a wellhead with flow-line valve and supply connections which were open. The flow-line valve at the house was closed and not active. The property owner, Chris Mitchel, has previously indicated that the well had been in active use for the house. Mr. Mitchel has not been responsive to attempts to direct him to establish proper registration.
