Drill/clean out has commenced.  Initial returns, as expected, are rubber and cement. We are now on top of the bridge plug, as indicated by the returns.  At this point, penetration rate is 4 inches per hour.   Have blown hydraulic line on swivel. Line repaired.  Service Master requesting rig shut down for one (1) hour while they make adjustments to tarps on the house.  Rig crew retightening lines and repairing a few small leaks.  Drill/cleanout resumes. RW Tree service on location to collect aerial photos of site.  Rig has shut down, and will resume drill/cleanout in morning.  Estimated depth is +/-180 feet.  (Did not want to drill through bridge plug this late in day as the possibility of a large pocket of gas is real.)
1434042025 2015-06-10 Orphan No Violations I inspected this location as part of an on-going investigation into the source of stray gas impacting the referenced home. A review of historical documents and maps suggests that a former Ohio Fuel Gas well may be located on this parcel. The home owner has reported that there has been a small dead spot near the front garden area for several years. A screening evaluation was performed with punch bar and hand-held gas meter which indicated elevated levels of gas at the dead spot. Based on these findings, the homeowner has contracted with Moores Well Service to perform an exploratory excavation at that spot with a small tracked excavator. A well was discovered at this location. The well is comprised of a 10-inch casing, which is open to atmosphere. The top of the casing is ragged and severely corroded with irregular edges and partially collapsed. Water can be observed within the casing at a depth of 10 feet. The surface of the water is actively bubbling with gas. No other casings are visible. The casing is open to a depth of at least 400 feet based on depth-finder measurements. There is some blackened and stained soils in close proximity to the well but there is no visible oil and oil is not present of the surface of the water inside the casing. Following collection of measurements and other documentation, a passive vent structure was constructed at the well using a plastic 30-gallon container and 2-inch diameter PVC for a vent stack which was affixed to the northeast corner of the house. The well was then re-buried pending further action. I will assist the homeowner in preparing an application for entry to the Idle and Orphan Well Program.
1447098919 2015-11-09 Orphan No Violations I revisited this location pursuant to contact from Mike Distin. Mike reported that he had started to observe elevated gas readings inside his house over that last couple of days. His reported combustible gas readings of 200 to 300 ppm sustained inside the house, with an occasional peak of 600 ppm. He reported that the higher readings tended to be closer to the front of the house, nearer the well. Until yesterday, Mike reported that he had not been seeing any detectable levels of gas.

I inspected the vent stack and found it to be venting gas. I also performed a general soil probe survey in the vicinity if the well. I detected a reading of 0.5% methane in a soil probe location at the well site. I did not observe detectable gas readings at any other locations. I advised Mike to contact the FD and seek their consult regarding indoor safety and confirm the gas readings he was observing. Olmsted Falls Fire Chief Munteanu and his assistant arrived and collected gas readings inside the residence which correlated well with Mike Distins readings. At the time of this inspection, combustible gas readings inside the residence are sustained around 230 ppm. I did not detect %LEL or % methane readings inside the house. Chief Munteanu advised Mike Distin to contact Columbia Gas to rule out line gas as a source. Otherwise, the FD did not deem the levels of gas to be an immediate danger. Mike is waiting to see if I&O can provide assistance.

The vent stack on the well is showing very little signs of natural gas being vented.
The contractor bailed a sample of water from the wellbore. A quantab was used on the sample and the results showed the water contained 80 ppm chlorides. The water sample is free of sediment and has good clarity.

1501861213 2017-08-03 Plug / Plug Back No Violations Rig crew on location. Mixed additional drilling mud and placed Sakrete in annular space to help stabilize 7-inch diameter casing. Drill/cleanout commenced. It is estimated that we are 50%-75% through the bridge plug. Progress has virtually stopped. Tripping bit out of hole to check. Tubing, weight sticks, and bit out of hole. No apparent issues with the bit. Will go back in hole in morning with a smaller bit to make pilot hole, then a tapered mil to finish off.
1502378760 2017-08-07 Plug / Plug Back No Violations Rig crew on location preparing to commence drill/cleanout. Well is gassing, crew mixing more mud.
Drill/cleanout commenced from +/- 192 feet.  At +/- 476 feet, hit something hard.  Crew drilled through obstruction with little difficulty.  At depth of +/- 508 feet have decided to circulate on hole for 30 minutes prior to resuming more drill/cleanout.    Have hit another obstruction at +/- 530 feet.  Returns are shale, and miniscule pieces of wood.

Progress stopped 30 minutes prior. Tripping out of hole to check bit. Depth is +/-535 feet. Tubing out of hole. Mil has twisted off and is on bottom.

C Pro unloaded and equipment being made ready. Boring commenced. Bore has been completed. Vault and vent lines installed, all ditches backfilled. Vault has been placed and secured. All fencing has been removed. Matting still in place, and scheduled for removal in morning.

Vegetative growth has been very well established and landowner has mowed yard several times. 2 sidewalk sections that were minorly damged have been replaced and landowner is satisfied. FINAL RESTORATION IS APPROVED.

Vegetation has been established in all of the disturded areas of the work location. The side walk has been repaired by the contractor. The vent line is operating properly. There was no odor of gas noted around the well location or coming from the vent line. The landowner had no complaints and is pleased with the work performed. Final restoration can be passed.

-465 feet to set bottom plug. Petro Set Cementing on location. 250 feet of bottom plug (465 feet to 215 feet,) placed with 100 Sacks of Class A cement. Rig crew has tripped 2.375 inch diameter tubing out of hole. Petro Set off location. Rig crew in process of disassembling flow lines from diverter and loading assorted lines onto trailer.Very small leak is noted. A check of the plug indicates the plug is at +/- 261 feet. After conferring with Mgr. Chini, he concurs with our decision to grout remainder of hole in morning. S & H Water Service Truck scheduled to remove 1 of the tanks off location at 2:00 PM.

1662592985 2018-03-08 Orphan No Violations I met with the homeowner (Mike Distin) per his complaint of observed low-levels of volatile organic constituents (VOCs) in ambient air, as measured from a hand-held portable gas metering device that he owns. The gas well that had been discovered in the front yard area previously, had now been plugged and abandoned under orphan well program contract. The P&A activity was completed in 2017. Since the completion of the plugging, the homeowner has reported observing levels of detected VOCs in the house, at various concentrations ranging from 20 parts per million (ppm) to 200 ppm. Subsequent to the plugging of the well, and in response to the observed VOCs, the homeowner had a sub-slab de-vaporization system installed by a contractor (A to Z Solutions). Since the installation of the system, the homeowner has reported that the observed levels of VOCs has not changed appreciably. During my inspection, I evaluated the area around the well, the well passive vent stack test port, and the house interior, and found no detectable gases (combustible gas LEL, CO, methane). I did not detect any gas smell or other unusual odors. Crews form Columbia Gas were scheduled to perform an assessment of the domestic supply to the house on the day of my inspection. I advised the homeowner that I would check back following the assessment by Columbia Gas.

2071937160 2017-07-31 Plug / Plug Back No Violations Various safety signs posted on location. Four (4) gas monitors placed inside residence. Four (4) monitors placed strategically on location. Commenced running 2.375 diameter tubing in hole. Sat down on unknown obstruction at +/-155 feet. Some necessary welding being done on flow lines. Appalachian Well Survey on location. Resumed drill out/clean out. We were able to get through the obstruction and are now at +/-

Tubing out of hole, rigging up Appalachian Well Surveyors. Have ran in to an unobstructed depth of 200 feet. Temperature log indicated gas at +/-50 feet. Ran 3 arm caliper log. Indicates that the hole is +/- 10 inch diameter from GL to +/- 150 feet. Hole decreases to +/- 8 inch diameter from +/- 150 feet to +/-200 feet. Ran and set a 6.90 inch diameter bridge plug at 180 feet.

Northeast Pipe and Supply arrived with 9 joints (+/-

Petro Set rigged up and pumping. Plug down after pumping 60 sacks Class A cement with 2% Calcium Chloride. Good cement to surface.

See daily report for details.8:30 AM: Inspector Kepler on location. Checked depth of plug in annular space, and found it to be +/-14 feet below top of hole. (Will top off on next plug setting.) A check with Sperian Gas Meter revealed no detectable gas readings.

Rig crew arrived with tubing for drill out and gel to assist in drilling. Rig crew will spend the day preparing to commence drill/clean out in morning. Well head will be cut off and another 7 inch diameter collar welded on to accommodate the flow diverter. Gel will be mixed and pipe tallied.
