1404924743 2014-07-07 Urban Drill / Deepen / Reopen No Violations I performed a routine inspection of the drilling location for this unit. At the time o f this inspection, rig-up activities have been completed and circulation pits are being completed. I observed the pit bottoms to be free of stones and obstructions and is dry. Liners are being placed and secured. Pit berms have been constructed and are stable. The lease road is dry and stable. Unconsolidated soils from the pit excavations has been bermed up on the west side of the drilling location as additional sound dampening and drainage control. During my inspection visit, drilling for the mouse hole has commenced. Competent shale bedrock material was encountered around 12 feet below grade. The cellar has been made liquid tight using plastic pit liner-type material. Drilling spud up will occur today.
1405019204 2014-07-10 Urban Drill / Deepen / Reopen No Violations I inspected the location of on-going drilling for this producing unit as part of routine site check. At the time of my visit the drillers are drilling on fluid at 2,850. The switch to fluid from air drilling occurred at 2,816 at 7 AM. At the time of my visit, drilling was proceeding at approximately 0.5 feet per minute. I observed the fluid level in the pit to be good, with ample freeboard and no observed oil. I observed the cellar to be holding fluid and free of oil. The driller reported no gas detections in the upper formations. Surface runoff appears to be draining well, the lease road is in good condition, and the soil piles are stable.
1407768245 2014-08-11 Urban Drill / Deepen / Reopen No Violations I inspected this unit following the recent completion of drilling activity. The former drilling fluid/recirculation pits have been solidified and filled. The former location of the pits has been graded and leveled. The remainder of work to complete preliminary restoration is pending the completion of current work-over and swabbing which is under way. Service rig crews are currently set up at the wellhead and are swabbing frac flow-back and production water into frac tanks staged on site. The well is producing some gas but is well controlled. Drainage around the well site is well managed. The lease road remains in good condition.
1410540084 2014-09-12 Plug / Plug Back No Violations I inspected this well location as part of plugging and abandonment activity which is commencing. The well was completed in 07/2014 and has not been placed into production. There is a wellhead only and no other production equipment. Rig up for plugging is set to begin within the next day.
1418143697 2014-12-09 Final Restoration No Violations I inspected this location as part of evaluation for final restoration. Attempts to place the well into production were unsuccessful and the well was plugged and abandoned on 09/11/2014. The former well location has been graded and seeded. The former pits are filled and re-graded. The access road, comprised of a gravel-paved road which extends into the location from Drake road, is still in place. I contacted Don Nething with Ohio Valley Energy about the road. I advised that a landowner waiver would be necessary if the road is to remain. He is checking on the what restoration strategy is planned.
1470753368 2016-08-09 Final Restoration No Violations I inspected this location as an evaluation for completion of Final Restoration. At the time of this inspection, I observed the following: The former locations of the well and drilling pits were level and graded to match the surroundings. Gravel-paved access road and associated well pad area remain based on landowner waiver. The well was never completed and, as such, no flow line or sales line appurtenances were ever installed at the location. Based on these findings, no further action is deemed necessary for Final Restoration.
1628620493 2014-07-08 Urban Drill / Deepen / Reopen No Violations I performed a status check as part of on-going drilling activity and test of blowout preventer (BOP). The BOP was installed and pressured up at 10 PM. The maximum test pressure was observed to be 540 PSI. Pressure was held for 30 minutes and the test was concluded with no observed problems.
1667297924 2014-07-08 Urban Drill / Deepen / Reopen No Violations I performed a routine site visit and inspection during on-going drilling activities. Upon my arrival, drilling had been completed to 469 below grade, from derrick floor (DF). The borehole is being circulated for surface casing. Last night, 11 conductor casing was set at 32 below grade (42 DL) and cemented in place. The surface casing program will consist of 442 of 23# 8.625 casing cemented to surface, and utilizing 5 centralizers. Casing was loaded and circulation established with no loss of fluid. Surface casing was cemented to surface with good returns. No further activity will be performed for 8 hours during cement curing and set-up. Blow-out preventer (BOP) will be tested at that time. Fluid levels in the pit are good with adequate free-board and stable liners. Rainfall during surface casing drilling and cementing was heavy at times, and site drainage appears to be well managed.
1914866554 2014-07-02 Urban Drill / Deepen / Reopen No Violations I inspected the location of the unit which is under construction at the time of my inspection. The lease road has been established, and is comprised of crushed limestone. The lease road entry point is off Drake Road, 550 feet east of Howe Road. The lease road travels north approximately 800 feet from the entry point, along the western boundary of I-71. The topopgraphic grade of the road is generally flat, with low-lying areas ot the immediate east and west. Earth work on the well pad area is on-going at the time of my inspection. Elements of the drilling rig (Wildcat) are being delivered and set up. The drilling rig is being oriented west to east. The drilling and circulation pits will be positioned on the south side. Signage is present at the head of the lease road. I met with Ted Hill of Ohio Valley Energy and we discussed adding a drainage swale along the south side of the pits if significant rain occurs. Surface drainage is being well managed with a low, temporary headwall berm on the west side of the pad to promote surface flow away from the residences. Ted also indicated that soil piles from the pits will be stockpiled on the west side of the rig for visual and sound dampening. Pits will be dug next week and spud will occur on or around 7/7/2014.
