0082059752 2018-06-19 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead and tank battery are located behind a single-family residence on Avery Rd. The area is comprised of two other producing units (Kral 1D and Kral 2D). The three wellheads share the common battery. Fittings and connections at the pump jack and battery are secure and free from signs of leakage or problems. The well was in production with active chart record. The interior of the secondary containment was clean and dry. Load-outs were plugged and gates are locked. Signage was present and the access road which extends east from the shared driveway was in good condition.
0225369489 2012-09-18 Urban Drill / Deepen / Reopen No Violations I was on site to inspect the cementing of the conductor casing. Wildcat Drilling Co. drilled a 17 hole on air to a depth of 37 ft. and placed one 42 ft. joint of 11.750 conductor casing. The rig pumped water through the pipe and established circulation to clean the hole of debris and dirt. Petroset Cementing arrived on location and set up equipment for cementing. The water used for mixing the cement was checked for proper pH and showed a pH of 7. Petroset pumped 4 bbls of water for circulation and I observed the water circulating to the surface around the conductor. Petroset mixed 120 sks.of Class A cement with 3% CaCl added and began pumping the cement. I weighed the cement with a mud scale and got a weight of 15.6 lbs. After pumping for approximately 20 minutes, I observed the cement return to the surface around the conductor casing. Petroset kept pumping until the cement was observed to be the proper viscosity. The conductor casing will have 5 ft. cut off of it when the cement has set up.
0333362995 2012-11-06 UrbanProduction Wells No Violations On my visit to the well today I was accompanied by supervisor Tom Hill. Status check found that well is not yet completed as it was just drilled this summer. Wellhead and pump jack is installed and company is proceeding with remainder of tank battery construction. It was recommended that a drainage pipe be installed in the northeast corner of the site proceeding to the southeast corner. Also recommended that some silt fence be installed to the southeast corner to insure no soil erosion occur in the low end of the site. I will follow up with an additional inspection within thirty days.
0677288316 2013-10-03 UrbanProduction Wells No Violations The producing unit consists of a pump jack unit at the wellhead and an aboveground storage tank (AST) battery. The battery is comprised of 6 140-barrel steel production tanks, three 25-barrel poly brine tanks, and separator units. The battery is shared with the Kral #1D and Kral #3D units. There is a Dominion metering run in the battery. The elements are enclosed in separate board-on-board enclosures. There is also a separately enclosed heater unit. There is adequate secondary containment. The signage is in good condition. Urban drilling process safety equipment is present. The load outs are locked and bull-plugged.
0832310896 2012-10-31 Administrative Inspection No Violations On this date I was accompanied by Supervisor Tom Hill and we travelled to this well for the purpose of obtaining water samples from the surface water in the back yard of Mr. Rick Hutchinson, 2176 Rusnak Trail, Broadview Heights, Ohio 44147. We were accompanied by Rick Reeve, Geologist and Mitch Ciccarone of Gonzoil, INC. We met very briefly with Mr. Hutchinson, explaining the testing that would be done. Tom Hill and I gathered water samples according to ODNR Policies and they were sent by UPS on this date at 3:00 PM.
0894757812 2013-08-30 UrbanProduction Wells No Violations I inspected this unit as a follow-up to an earlier complaint (#3361). Oil had previously leaked from the associated flow line and impacted an area of an adjacent property. Remedial action had been taken by the well owner and the oil and oil-impacted soils have been cleaned-up and the flow line problem rectified. I spoke with the original complainant and inspected the formerly impacted area and the flow-line. The area has been restored and the complainant is satisfied with the results. No further action is warranted.
0952850789 2012-09-19 Urban Drill / Deepen / Reopen No Violations I was on site to inspect the cementing of the 8.625 in. surface casing. Wildcat Drilling Co. drilled an 11 in. hole on air to a depth of 468 ft. and set 438 ft. of 8.625 in. surface casing using a 12 ft. section of landing pipe. Three centralizers were placed on the casing, one between joints 1 and 2, one between joints 5 and 6 and one on top of joint 8. Petroset arrived on site and set up the cement head with plug. Petroset then began to pump water for circulation and to clean the hole of dirt and debris. The water was tested for pH level and was found to have a pH of 7. I observed circulation return to the surface and Petroset began mixing and pumping 200 sks. of Class A cement with 3% CaCl. 50 lbs. of cottonseed hulls and 150 lbs. of gel were sent ahead of the cement. After pumping, I observed some of the cottonseed hulls and the gel return to the surface. I then observed cement return to the surface and Petroset kept pumping until the cement was at the proper viscosity. Petroset then pumped down the cement plug at 8:50 am at a pressure of 1200 psi..
1227570285 2017-02-01 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection I observed the following: The well is part of a three well cluster located in a a mostly wooded parcel. The unpaved access road and areas in and around the well were mostly snow covered, but were in good condition with no signs of erosional deficits or degradation. The production system was comprised of a pump jack and tank battery in separate board-on-board enclosures located in close proximity. The tank battery also serves the two other producing wells on this parcel (Kral 1-D and the Kral 2-D). The enclosure gates were closed and locked. The pump jack was in operation with no evidence of problems or leakage. I walked the interior of the tank battery enclosure and inspected the tanks and associated fittings, connections, and pipe runs. I did not observe any signs of leakage or spills. The tank load line valves were plugged and locked. The tank lids were closed. I walked the areas surrounding the wellhead and battery and found no evidence of impact or degradation.
1348180293 2012-09-17 Urban Drill / Deepen / Reopen No Violations I inspected this new drill site and observed Wildcat Drilling Co. Rig #1 moving in on location. At the time of my inspection, the rig mast was erected and the doghouse was in place. Several tractor trailer loads of pipe tubs containing drill collars and equipment were being winched into place. The v door assembly and pipe racks were also being put into place. Two 300 bbl water tanks were on the location. A port-o-let was also on location for the crew to use. The lease road was well maintained and had limestone spread on it. No mud was observed being dragged out onto Avery Road.
1348453162 2012-09-19 Urban Drill / Deepen / Reopen No Violations I inspected the blowout preventer (B.O.P.) testing procedure being done by Wildcat Drilling Co. Rig #1. When I arrived, the drilling crew had a pressurized air line hooked up to the B.O.P. stack and was preparing to pressure up the stack. I observed the pressure being brought up to 420 psi. I set a timer for 15 minutes and observed the pressure gauge on the stack. After 15 minutes I observed the pressure gauge holding at 420 psi.. I informed the crew that the B.O.P. stack had passed the pressure test at 4:45pm. A written note was made on the drillers log in the doghouse that the B.O.P. test passed along with the time.
1348576970 2012-09-24 Urban Drill / Deepen / Reopen No Violations The purpose of this report is to update the prior report on this well. A 7-7/8 hole was drilled to 3,821 and 4.50 casing was set to 3,795 using new, range 3, 10.5 pound per foot casing. Casing was cemented using Class A cement with 3% CaCl. Plug down at 5:00 PM under 1500 pounds pressure. Job completed with no issues, incidents or problems. Wildcat drilling to begin rigging down process to prepare move to McNab well in Cuyahoga County.
1353080403 2012-11-06 UrbanPreliminary Restoration No Violations On my inspection of the well site on this date I was accompanied by Supervisor Tom Hill. This well is not yet in service. The pump jack is installed and preparations are in place to complete well construction. The access road is paved with asphalt for the first 100 and then limestone graded and packed the remainder of the way. The site is graded and recommendations were made to install a drain tile in the northeast corner of the site for 30 - 40, running north to south to absorb water runoff. It was also recommended that erosion control silt fence be placed in the south east corner until the soil and grass become stable on the site. Preliminary restoration inspection is not passed at this time. A follow up inspection will occur in 30 days to check on restoration and completion status.
1354204803 2012-11-27 UrbanPreliminary Restoration No Violations My visit to the well today was to perform an Urban Preliminary Restoration inspection. Well site has been constructed to include the tank battery. The wellhead area is neat and clean and no violations are noted. The area is enclosed in chain link fence and secured by locks. The tank battery is a shared location with the KRAL 1D and the KRAL 2D wells. There are six 140BBL storage tanks and three 70 BBL brine tanks. Each unit has its own tank system The caps and vents are secure and there is a charcoal filter in place. There is one horizontal and one vertical separator for this well, with a metering run and oil pick up lines which are secured with locks and bull head plugs. The access road is packed crushed limestone to a depth of six inches. Due to the lateness of the seasons, seeding for this well is not complete and Urban Preliminary Inspection is not passed. A follow up inspection will be conducted in the spring for restoration completion. It was recommended that a drain tile be placed approximately twenty feet in the northeast corner running north to south to carry any excess water away from that corner of the well site.
1355245611 2012-10-30 Urban Drill / Deepen / Reopen No Violations On 10/29/2012 I received a call from Mr. Rick Hutchinson, 2176 Rusnak Trail, Broadview Heights, Ohio. Mr. Hutchinson was requesting that I test Black water laying in his back yard for oil field contamination due to the recent drilling of the nearby oil well behind his home. This was the Kral 3D well which completed drilling on 9/24/2012. Mr. Hutchinson informed me that he had contacted Doug Gonzalez of Gonzoil, INC over the weekend and that Gonzoil employees had obtained a water sample at that time. I contacted Doug Gonzalez who stated that if a problem from the well was found he would alleviate the problem to the satisfaction of ODNR. Upon investigation of Mr. Hutchinsons yard I discovered water laying on the ground approximately two inches in depth. Near a tree island in the yard I discovered a partially exposed plastic drain tile. I was unable to determine any black water in the area. After a discussion by phone later in the day, I again traveled to Mr. Hutchinsons home where he showed me the area in question. At that time I noted a dark area of water near the drain tile. The water had a dark brown area which I took photographs of. I informed Mr. Hutchinson that I would contact my supervisor and we would return the next day and obtain water samples as I did not have sampling materials in my vehicle. During my inspection I could not see any visual appearances or evidence of any oil contamination anywhere between the well site or on Mr. Hutchinsons property.
1363108525 2013-03-12 Urban Drill / Deepen / Reopen No Violations This report is being issued in response to a complaint filed on 9/24/2012. Laurel Luke filed a report concerning the presence of fumes created by the exhaust from the drilling rig being used to drill this well. Also, the complainant complained about the noise from the drilling rig, stating that it caused her to tremble and it was hurting her ears. She demanded that the rig be required to install quieter mufflers. I explained to her hospital grade mufflers and that she should contact her legislators. She stated that this would be a waste of her time. Further inspection showed her home to be outside the 500 radius from the well. I have had no further contact from her following this phone call. Complaint #4040 should be closed.
1499933103 2012-12-11 Production Wells No Violations On this date, I received a phone call from Wendy Booth, Program Administrator 1, ODNR requesting that I contact Broadview Heights, Ohio councilman Tom Pavlica concerning the fact that Mr. Hutchinson had not yet received results of the water testing done on Mr. Hutchinsons yard located at 2176 Rusnak Trail, Broadview Heights, Ohio 44147. The following is a time line of the sampling since my report of 10/31/2012. The sample taken on that date was received by the lab on 11/01/2012 by Jason McClarren. On 11/20/2012 the results were returned to me via e-mail from Wendy Booth. On 11/27/2012, Steve Tompkins reviewed the test results and referred the summary to me. On 11/29/12 a request for Kyle Tharp, Hydro Geologist to review the results of the testing. The results of the testing were invalid and therefore are not usable. I informed the councilman I would have a letter to him by this weekend.
