The well and tank battery are located in cleared, but undeveloped parcel in an otherwise residential area, adjacent to the house at the referenced address. Fittings and connections at the wellhead pump jack and battery are secure and free from signs of leakage or problems. Load-outs are secure. Gates are locked. Process safety equipment is present. No problems noted at the time of this inspection.
0061505168 2017-04-27 UrbanProduction Wells No Violations I inspected this producing well location as part of rotuine inspection activity. At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate board-on-board enclosures. The enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The surface casing pressure was being monitoed and showed no pressure. The electrical panel box was locked and secure. Fittings and connections at the battery were secure. The load-line vlaves were plugged and locked. The tank lids were closed and sealed. The secondary containment drain was secure. There was some standing water in the containment area from recent rainfall. The water was clear with no signs of floating oil or sheen. The gravel-paved accesss road was level and dry. I did note some landscaping fabic debris in the parking area and I notified CJ Cutter about the need for removal of the debris. The entry gate was locked. I walked the areas surrounding the well and battery systems and observed the natrual soils and vegetation to tbe free from signs of leaks, spills, or other adverse impact from the on-going produciton activity.
0237951773 2019-05-01 UrbanProduction Wells No Violations The tank battery floor had standing storm water in it that needed to be removed. The Dike around the tank battery has eroded down allowing a gap between the dike and fence. The dike needs to be built up to close the gaps. No violations found. All locks, safety equipment, and fencing around well head and tank battery have been installed. Proper identification was posted at well and tank battery. Tank battery is shared by well number 2-2147

Following maintenance issues must be addressed: 1. Water should be drained from tank battery. 2. Dike should be built back up around tank battery and should be monitor for erosion.

0576152778 2017-11-08 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well and associated battery are located on an otherwise undeveloped parcel, and the battery is shared with the Callas #5 well located on the same parcel. At the time of my inspection, the fittings and connections at the well were secure. The tank lids were closed and secure. The load-line valves were plugged and locked. The secondary containment was intact, with some water from recent rains. There was no signs of leakage (floating oil or sheen) visible. I walked and inspected the areas surrounding the wellhead and battery and found no signs or evidence of impacts or degradation. The access point was gated and locked with signage present. The access road was in good condition.
0937669276 2015-05-07 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. The well is located in a clearing within a primarily wooded area. The entry to the well is gated with a cable-style gate which is locked. The access road is gravel-paved and accesses two wellheads (Callas 5 and 6D). At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of four 140-barrel steel production tanks, one 100-barrel steel brine tank, and two mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. ID signage is needed at the entry gate. Evergreen screening is needed at the battery and wellhead. Surface runoff from the battery area to the north towards a small creek has been altered resulting in ponding. This should be properly managed with a swale or similar engineering remedy. Signage is present at the battery but lacking complete information. There is no signage at the wellhead. The 8.625-inch casing is not exposed for monitoring as required. In addition, the following safety issues were noted. Load lines are not locked. Production tanks are missing lightning arrestors. Brine is actively leaking from the 2-inch flange at the brine tank. The dike drain has been left open. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this matter.
1055900879 2015-01-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead pump jack enclosure is secure. Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow. The dedicated access road has been plowed and is passable. The tank battery which is shared with the #2-2147 unit is secure. Valves and connections are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.
1071784235 2012-11-28 UrbanPreliminary Restoration No Violations My visit to the well today was to perform an Urban Preliminary Restoration inspection. The access road is concrete for the first 30 feet and then packed crushed limestone and brick for the remainder of the well site. The wellhead is enclosed by a board on board fence and the gate is secured by lock and chain and is secure. No problems or leaks were noted at the well head. The tank battery is a shared location with the Callis 5 well, P# 2-2147 and the gate is secured with lock and chain. The dike surrounding the tanks is sufficient to hold fluids per 1501:09-9-05. There are two 140 BBL tanks for this well with caps closed and secure with vents and a charcoal filter in place. There is one 100BBL brine tank which is shared by both wells. This well has one vertical and one horizontal separator. Load lines are enclosed within the tank battery and are secured with locks and bull plugs. Urban Preliminary inspection passed.
1072619929 2018-10-17 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection and evaluation of current condition following recent change in ownership. At the time of this inspection, I met with EOS Energy Representative Mike Corp and North Royalton Fire Chief Chegan. I noted that the well was in active production. The enclosure gates were locked and secure. The identification was current and accurate. The load-line valves were plugged and locked. Tank lids were closed and sealed. I walked the areas surrounding the well and battery and found no signs of problems or indications of impact. I noted the following maintenance items that required attention: The gate at the access road was not secure and required proper signage. He indicated that he would have the issues addressed as soon as possible.
1185383125 2015-08-24 UrbanProduction Wells No Violations I inspected this producing unit as a follow-up to an earlier inspection in which several safety deficiencies had been noted. Those deficiencies consisted of the following: unsecured tank lids, unlocked load-lines and electrical boxes, incomplete or damaged ID signage, open containment drain, and missing spark arrestors on the production tanks. At the time of this inspection, the noted safety deficiencies have been addressed. The 8.625-inch casing is plumbed to surface with pop-off. The previous inspection had also noted some brine leakage at the poly tank. At the time of this inspection, these issues have been addressed. I have also discussed with the owner the on-going management of surface water runoff adjacent to the wellhead. This area had been previously noted to be forming a natural run-off feature and may possible need managed to prevent erosional soil loss. At the time of this inspection, natural vegetation has become established following a previous high-flow event. The need for management does not appear warranted at this time.
