At the time of this inspection, the fittings and connections at the wellhead and battery were secure. Tank lids were closed and secure. Load-lines were locked and secure. The chart record was active. Production reports were current. Enclosure gates were locked. I walked the areas surrounding the well and battery and found normal conditions of the extant greenscape. The screening that had recently been planted along the west side of the battery appeared to be thriving.

At the time of this inspection, the load-lines were locked and secure. I inspected the areas and around the battery and wellhead and found no signs of problems or impacts. The evergreen screening along the west side of the battery remains partially missing pending replacement.
0963459155 2015-11-25 UrbanProduction Wells No Violations I inspected this well location as a follow-up to earlier inspections in which it had been noted that the screening at the tank battery had been severely impacted by deer predation. Based on these observations, the owner/operator agreed to replace the screening with a different species of tree (spruce) in an effort to avoid the deer grazing problem. At the time of this inspection, the screening tress on the road side have been replaced. Don Nething with Ohio Valley Energy reported that the land owner had requested that the arbor vitae tress on the south side be left in place, so that he could try to rehabilitate them.

1204088429 2017-04-27 UrbanProduction Wells No Violations I inspected this producing well location as part of routine inspection activity. At the time of this inspection, I noted the following: a wellhead with a linear rod pump (LRP) system and associated tank battery in separate board-on-board enclosures located on adjacent residential parcels. The well and battery enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. There was some standing water in the containment area from recent rainfall. The water was clear with no signs of floating oil or sheen. The well and battery are both accessed from the shared-use driveways for residential parcels. These access-ways were in good condition. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity. I noted that the recently-planted evergreen trees on the west side of the battery were showing some signs of stress. I notified Don Nething about the tress.
1302541277 2011-04-11 Drill / Deepen / Reopen No Violations Operator is currently rigging down. Operator has temporary seeded topsoil stockpiles on location. Drainage controls were in place. Lease road was stable. Pit fluid level was sufficient. Fencing was up around the pits and the location.
1304602230 2011-04-07 Drill / Deepen / Reopen No Violations Phone call from Mark Spicer (North Royalton Engineering Department) stating there is off site runoff occurring from the stockpiled top soil along the east side of the site. I asked DNR Inspector Tom Hill to asist me on site. The silt fence along the north perimeter blew out due to the heavy rain that occurred on the 4/4/11. This off-site runoff affected a pond on the adjoining property which turned water a brown/tan color. I advised company (Dale Nethings) to enlarge the existing sediment pond and add overflow siphons. I also advised that he construct an additional sediment pond to the north perimeter line. The additional work was completed on 04-08-2011 and DNR Inspector Tom Hill checked the work.

Company is adding a third well to this location. I also inspected this well and found no violations. Well is in compliance. I was accompanied by Richard Bayless from the Columbus office.
1353087476 2012-11-06 UrbanPreliminary Restoration No Violations On my inspection of this well for urban preliminary restoration I was accompanied by supervisor Tom Hill. The wellhead is equipped with a Linger Rod pump and is incased in a board on board fence and secured with locks. The pressure on the annulus is 200 PSI. The interior of the pump location is clean and free of debris. The tank battery consists of one 110 Barrel lowboy tank, one 50 barrel brine tank, a separator and a metering run. There are two load lines exiting the rear of the tank battery and are secured with bull plugs. Preliminary inspection passed.

The well is comprised of a linear rod pump system in a board-on-board enclosure. The well enclosure is locked and secure. Fittings and connections are secure and free from signs of leakage. The battery is located in a separate board-on-board enclosure which is locked. Safety equipment is present. Tank hatches are secure. The interior of the battery is dry and free from signs of leakage or problems. One of the external load lines is not lock protected. The electrical box requires a lock also. The ID signage requires additional information to be compliant. The owner must correct these deficiencies as soon as practicable or by 07/03/2015. Please contact Inspector Stephen Tompkins regarding this inspection.
1395764523 2014-03-25 UrbanPreliminary Restoration No Violations The unit is comprised of a wellhead with a linear rod pumping unit in a board-on-board enclosure and aboveground storage tank (AST) battery with 1 100-barrel production tank and 1 50-barrel poly brine tank. The unit is producing. The loadouts are locked. There is secondary containment at the battery, ID placards at the wellhead and battery, and process safety equipment (low-pressure relief valve, spark arrestor, and carbon filter). Evergreen screening is present around the battery and screening has been placed at the top of the driveway/lease road. The internal spacing at the battery is in compliance with the rules. The 8.625-inch casing is currently buried. I contacted Ted with Ohio Valley Energy and advised him that the backside must be exposed and gauged or valved.
1400329254 2011-04-08 Drill / Deepen / Reopen No Violations Drainage controls have been installed. Operator has installed silt fence on the north and south side of the lease road before the home. Two sumps have been constructed, one at the south end of the drilling location, where the 36 inch culvert empties onto the property and a 2nd sump at the north end of the property. The north sump is where the site discharges off of the property. Both of the sumps had three 8 inch siphons installed in them. Site had received heavy rain that night, the rain washed grease and oil from the air trailer. Operator placed diapers and a boom around the trailer to collect the residual oil and grease coming from the trailer. Pit; the north end of the pit the liner was not visible water level was up to the berm. Physically confirmed that the liner was in place under the berm and water. Driller had water trucks on location and ordered to remove water from the water tanks. Pit water was pumped into water tanks to allow for additional free board. Lease road was stable. Drill depth was 2189 feet. BOP was in place and chained down.

The wellhead is located on the northeast side of the residence located at 10229 State Road. The tank battery is located near the road and is accessed from the neighboring drive on the north side. The wellhead and tank battery are located in separate board-on-board enclosures. The fittings and connections are secure and free from signs of leaks or problems. Secondary containment is clear and dry, Load-outs and gates are locked.
1447364996 2015-11-12 UrbanProduction Wells No Violations I inspected this producing unit as a follow-up to an earlier inspection in which I had observed the evergreen screening at the tank battery to be stressed and not likely to survive, as a result of deer feeding activity. Ive had several subsequent discussions with the Ohio Valley Energy regarding possible alternatives. At this time, Ohio Valley reports that they are securing bids from landscaping firms to replace the trees. Don Nething of Ohio Valley reports that they should be prepared to award the work within the next week.
1510150088 2017-11-08 UrbanProduction Wells No Violations I inspected this well location as a follow-up to on-going efforts being conducted by the well owner to remedy problems with maintaining the evergreen screening at the tank battery. At the time of this inspection, Ohio Valley Energy had recently replanted a portion of the screening using a smaller, deciduous shrub in place of the evergreens (pines, and Arbor Vitae) that had experienced deer predation and other problems in the past. I inspected the various elements of the production system and found the fittings and connections secure. Enclosure gates were secure with no signs of problems. I will continue to evaluate the viability of the shrubbery over the fall and winter months.

1597006440 2011-04-07 Drill / Deepen / Reopen No Violations On site to assist inspector Norburt Lowder. Met Michael K. Fabish Fire Chief for North Royalton, Chief received a call from the Mayors office to review site. Reviewed drainage controls on site. Operator had installed drainage controls in one location on site, drainage channel to the north of the drill location. Silt fence and straw bales were not installed correctly. Silt fence was not trenched in. Straw bales were not staked in. Drainage moved straw bails. Pond to the north of the drill location was brown in color. The stream that receives drainage from the pond was cloudy. Instructed operator to install silt fence on the north and south side of the lease road, from State road to the home. Install silt fence around topsoil stockpiles north of the home, down slope side. Install a sump with a siphon dam at the north end of the location. Repair silt fence in drainage channel and stake down straw bails or remove straw bales. Reviewed with equipment operator the correct way to install silt fence and straw bales. Mark Schmitzer City Engineer arrived on site, reviewed updated drainage plan with Mark. Mark requested a 2nd sump be installed at the north end of the drill location. A culvert discharges onto the property at that location. Drainage flows south to north across the drill location. Drainage flows from State Road east across the front of the location. Lease road was stable. Safety fence was up around pits and site.

1879556790 2017-06-15 UrbanProduction Wells No Violations I inspected this well location along with DOGRM Asst. Supervisor Stephen Ochs as a follow-up to a complaint reported by the City of North Royalton. The complaint was regarding the absence of screening vegetation on the west side of the tank battery. At the time of this inspection, we found the west side of the battery was missing screening in places. The owner has reported that the recently-planted pine tress were struggling to survive and some other types of screening were being evaluated. The arbor vitae that comprised the original screening, was subjected to predation by the local migrant deer population, which resulted in denudation of the foliage form the lower portions of many of the tress. The company is actively seeking alternative screening and have pledged to have new screening in place by autumn of this year. We inspected the active production equipment and found the following: The wellhead linear rod pump was in operation. The fittings and connections were secure. Secondary containment at the battery was intact and dry. We walked the areas around the well and battery and found the natural greenscape to be free from signs of impact associated with the on-going well operation. Production reports were current and there were no other problems or issues noted.
