The wellhead and battery are located in separate board-on-board enclosures which are locked. The battery consists of 1 140-barrel production tank and 1 50-barrel brine tank, and separator. Secondary containment, signage and spacing are in compliance. The unit is producing and no problems observed.
0569742448 2014-05-22 UrbanPreliminary Restoration No Violations I inspected this producing unit to evaluate conditions for preliminary restoration. The well was completed in 04/30/2010. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is valved to surface but is not gauged. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located northwest and adjacent to the wellhead. The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel poly brine tank, and a horizontal separator. The battery is located in a separate board-on-board enclsoure which is gated and locekd. The load-outs are bull-plugged and locked with clamshell-style secondary containment devices. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area formerly occupied by drilling equipment, including former pits, has been restored and reclaimed. There is well established grassy vegetation throughout the area. The area of the well is moderately sloping with a topographic drop to the southwest. Surface draininage appears to be to the west. There are no surface water courses evident near the site. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a shared drivway with the house at 9216 Highland Drive . The driveway is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are moderately wooded with no clear view of other homes. Requirements for preliminary restoration are met.

The well and battery are located on a rural, residential parcel with a shared-use, gravel-paved access road/driveway. The access road was in a good condition and passable. The enclosure gates were locked. Load-lines were locked and secure. Tank lids were closed and secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. There was an active chart record and production reports were current. I walked the areas surrounding the well and found no signs of problems or impacts relating to the production operation.

The well and battery are located in a clearing in a wooded parcel which is shared with a cabin-style home Access to the well is afforded by a gravel-paved access road which enters from Highland Drive and proceeds west to the house and wellhead. At the time of this inspection, I observed the following: enclosure gates are locked and secure. Tank hatch lids are closed and sealed. Load lines are lock-protected and secure. Fittings and connections at the wellhead and battery are secure and free from signs of leakage. Process safety equipment is present. The secondary containment is intact and free of standing water or staining. The entry road is well-drained and free from erosional deficits. The ID signage is not compliant with OAC 1501:9-9--e). I notified Bass regarding the ID signage. Bass has reported that new signs are being prepared.
