The wellhead area is neat and clean with no violations found. The well head is encased in a board enclosure and is secured with locks. The tank battery is enclosed in a boarded fence and the is clean with no obstructions. There are two 140 BBL storage tanks with caps which are closed and secured. Each tank has a vent system. A charcoal filtering system is in place. There is one 35 BBL brine tank. There is one vertical and one horizontal separator in the unit. A metering run is in place. Pick up lines are inside unit and is secured with bull head plugs. The access road is located at 8768 Falls Lane on the left hand side. The first 100 feet is asphalt paving, then crushed limestone for the remainder. No violations were found and Urban Preliminary Restoration passes.

1421332672 2015-01-14 UrbanProduction Wells No Violations I inspected this producing unit pursuant to a complaint registered by a nearby resident. (Reference Complaint #4912). The complainant reported a loud squealing sound coming from the wellhead area. The complainant reported that the sound had been occurring for a couple of weeks. The producing unit is comprised of a wellhead with pump jack unit and tank battery is separate board-on-board enclosures located in an area of single-family homes. Access to the unit is from Falls Lane along a vacated street easement which travels west 200 feet and terminates. The well and battery are located near the terminus. Upon my inspection, I observed the wellhead to be quiet with stable and secure valve fittings and connections. During my inspection the pumping unit began a pumping cycle and I observed the drive belts to be laboring and squealing on the spindle during each rotation. I called CJ Cutter and apprised him of the observed problem . He indicated that he would have a field person go and check the unit. I did not observe any other problems. Connections and fittings at the tank battery are secure and free from signs of leakage or problems at the time of this inspection.
1423580719 2015-02-10 UrbanProduction Wells No Violations I inspected this producing unit as a follow-up to a previous complaint and subsequent inspection on 01/14/2015. The well had been reported by a neighbor to be emitting a loud noise. Upon my last inspection I observed the drive belts at the pump jack to be squealing during operation. I notified CJ Cutter about the problem. At the time of this inspection, I observed the operation of the pump jack and found it to be functioning normally with no abnormal noises.
1431700506 2015-05-08 UrbanProduction Wells Violations Noted I inspected this well location as part of routine inspection activity. The well is located in a single-family residential neighborhood. There is a short access road from Falls Lane which terminates at the wellhead. At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The tank battery is comprised of a total of two 100-barrel steel production tanks, one 50-barrel poly brine tank, and mechanical separator. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. There is secondary containment but there is no berm on the north side allowing surface runoff to enter the battery. The 8.625-inch casing is not exposed at the surface to facilitate monitoring as required. Signage is present but lacking complete information. There is discarded refuse in the wellhead enclosure which should be properly managed. The tank battery requires two doors on opposites sides. An empty 55-gallon drum and chemical pump should be removed if not used. Carbon filters must be periodically evaluated for properly operation. In addition, the following safety issues were noted. Load lines are not locked. Brine is actively leaking from the 2-inch flange at the brine tank. The drain for the battery secondary containment has been left open. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this Compliance Notice.
1550685591 2019-02-19 Production Wells No Violations On Location to investigate Complaint # 6077, that was received on 02-15-2019 Regarding noise from the well. The well that is emitting a loud noise is approximately 380 feet to the northwest of the landowner. On 02-15-2019 I phone Ed Cutright from EOS Energy, to respond to the site and check out the issue of noise. I was informed by CJ Cutter that the pivot bearing needed greased. At the time of this inspection, the pump jack was not in operation. No compliance issues were found.
