The unit shares the TB with the #2177 unit. The spacing is in accordance with the rule. The well head is located 60 feet north and is fitted with a plunger lift. I did not observe any signs of leaking product or gas vapors. The ID is visible and good. The gates are locked. The tank battery dike is adequate. No violations noted.

The wellhead mechanical plunger lift and tank battery are in separate chain-link enclosures. The gates are secure and locked. Load-outs are plugged and locked. Process safety gear is present. ID signage is present and accurate. Fittings and connections are secure and free from signs of leakage or problems. At the time of this inspection, there is 12 to 14 inches of snow cover which obscures some surficial features.
1097862360 2017-11-22 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and battery are located on a working farm parcel, with the battery located next to the gravel-paved dedicated access road, and the well beyond. The battery is shared with the #2-2177 well, located north, across Ridgebury Road. At the time of this inspection, I found the following: a wellhead plunger lift and associated battery in separate chain-link enclosures. The fittings and connections were secure and free from leakage or problems. Fittings and connections at the battery were secure. The load-lines were plugged and locked. ID signage was present and current. The secondary containment was intact with no signs of leakage, spills, or problems. The well was in active production. The chart recorder was active. Production reports were current. Bonding and insurance was in place and current.
1380724637 2011-09-24 Urban Drill / Deepen / Reopen No Violations FOLLOW UP TO NOV # 1948330034 On 9/24/2011, DNR Inspector Norburt Lowder received a call from Wade Balser of OEPA regarding a complaint he received from the Broadview Heights Fire Department (BHFD) of oil bubbling up in the back yard of 8490 Wyatt Road in Broadview Heights, Ohio. This property is owned by Irene Krotz and is located immediately south of the Kral 1D (#2187). DNR Inspector Lowder then contacted DNR Supervisor Gene Chini and DNR Inspector Ray Wallace, Gonzoil and Mrs. Krotz about the problem. DNR Inspector Wallace arrived at the site at approximately 5:15 PM on 9/24/2011. He inspected the back yards southeast of the well site, ( Krotz, Tremblay and Benes), and discovered evidence of petroleum on the grass in a small depression that ran through the Krotz and Tremblay back yards. He then investigated the Kral 1D drilling site and noticed that the soil adjacent to the drilling rig air compressor and mud pumps was covered with PeatZorb. There were small amounts of motor oil on the ground surface which appeared to be flowing into a small pool of water that had collected on the east side of the location. DNR Inspector Wallace also discovered some broken clay field tile adjacent to this pool of water and, upon further investigation, the opening to the solid clay feeder tile which appeared to run south along the east edge of the excavated portion of the drill site. It appears that the motor oil had made its way to the pool of water and into the drain tile. The well owner excavated two (2) holes by hand to locate the field tile outside of the drilling location. OEPA had straw bales placed around the second (farthest) excavated area and approved that work be terminated until Sunday, 9/25/11, due to darkness. On Sunday four (4) additional holes were excavated to locate tile and extent of impact. They eventually located the end of the field tile in what appeared to be a French drain located on the property line between Benes and Tremblay. There was a second drai

1806258888 2016-02-04 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: Ingress and egress from the well area is afforded by a dedicated lease road which enters from Ridgebury coincident with the house driveway at the referenced address. A wellhead with mechanical plunger lift system is located in a chain-link enclosure in a wooded portion of the lot. The well enclosure was locked and secure. Fittings and connections at the wellhead were secure and free from leakage. The 8.625-inch casing annulus was exposed and fitted with pressure-relief device. Areas around the wellhead consist of deciduous woods and woodlands vegetation, which was free from signs of stress or impact. The tank battery is located 50 feet south of the wellhead within the same grassy and wooded yard area. The battery is comprised of two 100-barrel, steel production tanks, two 50-barrel poly brine tanks. The battery is shared with the #2-2177 (DAlessandro #1). At the time of this inspection, the access gate to the battery was locked and secure. Signage was present. Fittings and connections were secure and free from signs of leakage or problems. Process safety gear and carbon filter system was present. Secondary containment was intact and dry. Tank lids were closed. The load-outs were plugged. Evergreen screening is present surrounding the battery and in good condition. I walked the area surrounding the battery and observed normal grassy vegetation with no signs of erosional deficits or impacts associated with the production operation. I did observe the following maintenance issue: the production tank lids are missing the closure wing-nuts. This needs replaced to effect a proper seal. The load-out valves are missing locks. I notified the well owner about the need for locks and closure wing-nuts.
