At the time of this inspection, I observed the following: a wellhead with plunger lift system and associated tank battery located on an open portion of an otherwise residential parcel. The wellhead and battery were in separate board-on-board enclosures which were locked. Access to the battery and wellhead was afforded by a dedicated, gravel-paved access road. The battery was comprised of one 140-barrel steel, low-boy production tank, one 50-barrel steel brine tank, and horizontal separator. The fittings and connections at the wellhead and battery were secure and free from signs of leakage. The load-lines were plugged, locked, and secure. The ID signage was present and accurate. I walked the areas around the wellhead and battery and found no signs of impact or degradation. |

Fittings and connections at the wellhead and battery are secure and free from signs of leakage. The secondary containment is intact and free of standing water or staining. The entry road is well-drained and free from erosional deficits. The ID signage is not compliant with OAC 1501:9-9-05(E)(1)(a-e). I notified Bass regarding the ID signage. Bass has reported that new signs are being prepared. |

Elements of the well consist of a production wellhead with standard valve assembly with mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. The well is currently being produced from the 4.5-inch casing and the 1.5-inch production tubing. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located west of the wellhead along the same access road. The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel steel brine tank, and a horizontal separator. There is also a Dominion metering run within the containment area. The battery is located in a separate board-on-board enclsoure which is gated and locekd. The load-out lines are bull-plugged and locked. The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery which is in good condition. The wellhead and battery are located in a topographically low-lying area north of the residence at Surface draininage from the producing unit appears to be to the north towards lower lying areas. There are no active streams or other water courses near the unit. I did not observe any signs of gully formation or washouts. The extant lease road consists of a dedicated gravel-paved drive just north of the house and driveway Thyere is a small turn-around between the tank battery and well . The lease road is currently in good condition and free of washout or erosional deficits. |
