0758263057 2015-03-30 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The wellhead and battery are located behind the single-family residence at the referenced address. Access to the well and battery are from the shared driveway and then continue west on the unpaved access road to the unit. The wellhead is comprised of a pump jack system. Fittings and connections are secure. The tank battery is comprised of 1 100-barrel steel production tank, 1 50-barrel poly brine tank and separator unit in a separate board-on-board enclosure. Elements of the system are in general good repair. Fittings and connections are secure and free from signs of leakage at the time of this inspection. The load-outs are plugged and secure. ID signage and secondary containment are present. Areas surrounding the wellhead and battery are reasonably well drained and free from signs of adverse impact from the on-going production activity associated with this well.

2016908144 2015-07-28 UrbanProduction Wells No Violations I inspected this well location pursuant to a complaint (#5087) regarding loud noise reported to be coming from the well location. At the time of my inspection, the pump jack is idle. I previously had notified Ohio Valley Energy regarding the noise complaint. Fittings and connections at the wellhead and the battery are secure. I did not observe any unusual noises. I will follow up again with the well owner to confirm that the source of the noise has been identified and resolved if associated with the well operation.
