0264158586 2014-07-16 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 09/28/2010. The unti is producing and production reports are current. Elements of the well consist of a production wellhead with standard valve assembly and pump jack system. The wellhead is located in a closed and locked chain-link enclosure. There is oil leaking from the stuffing box and staining the interior ballast area around the wellhead. The aboveground storage tank battery is located south of the well, along the access road. The battery is comprised of a total of 3 steel 140-barrel production tanks, 50-barrel poly brine tank, and horizontal separator. Spacing from well to battery, and spacing of elements within the battery meet regulations. The battery is located in a separate chain-link enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. I contacted Bob Allen with Ohio Valley regarding the oil leak at the stuffing box.

The well and associated battery are located behind the house at the referenced address, and accessed via a shared use drive. At the time of this inspection, fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. The load-lines were plugged, locked, and secure. Secondary containment was intact and secure. The individual enclosures were locked and secure. The access road was well drained with no signs of wash-outs or erosional deficits. ID signage was present.

Called Mr. Herbst and advised him of what is going on near him and that the State regulation supersede the city ordinance on noise at night.

1605162780 2018-03-22 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well and battery are co-located on a parcel occupied by the Cutter Oil Co. well #2-2135 (Pine 2D) . At the time of my inspection, I found the following: wellhead with pump jack system and associated battery in separate chain-link enclosures. The enclosure gates were locked and secure. The fittings and connections at the wellhead and battery were secure and free from problems, with the following exception: I noted that the stuffing box was oily and needing service, which I made notification to Ohio Valley. The well was in active production with current production reporting. The interior of the battery, including secondary containment was free from free oil, staining, or other indications of spills, leaks, or problems. The electrical panels and load line valves were locked and secure. I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity.
