The wellhead and battery are located behind the single-family residence at the referenced address. Access to the well and battery are from the shared driveway and then continue west on the unpaved access road to the unit. The wellhead is comprised of a pump jack system in a locked chain-link enclosure. Fittings and connections are secure. The tank battery is comprised of 2 100-barrel steel production tanks, 1 50-barrel poly brine tank and separator unit in a separate chain-link enclosure . Elements of the system are in general good repair. Fittings and connections are secure and free from signs of leakage at the time of this inspection. The load-outs are plugged and secure. ID signage and secondary containment are present. Areas surrounding the wellhead and battery are reasonably well drained and free from signs of adverse impact from the on-going production activity associated with this well.

The well and battery are located behind a single family residence, and accessed via a shared-use driveway. I found the wellhead and tank battery connections to be secure and free from leakage or problems. The load-lines were locked and secure. Enclosure gates were locked and secure. Production reports were current. The well was in production with an active chart record. I walked and inspected the areas surrounding the well and battery and found no signs of problems. There were several inches of snow in the area.
1067928238 2014-06-10 UrbanPreliminary Restoration No Violations I inspected this producing unit to evaluate conditions for preliminary restoration. The well was completed in 10/20/2012. Elements of the well consist of a production wellhead with standard valve assembly and pump jack . The wellhead is located in a closed and locked chain-link enclosure. The 8.625-inch casing is valved to surface but is not gauged. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located south and adjacent to the wellhead. The battery is comprised of 2 steel 100-barrel production tanks, 1 50-barrel poly brine tank, and a horizontal separator. The battery is located in a separate chain-link enclosure which is gated and locked. The load-outs are bull-plugged. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. There is a plastic drain tile which is plugged. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area formerly occupied by drilling equipment, including former pits, has been restored and reclaimed. There is well established grassy vegetation throughout the area. The area of the well exhibits a moderate topographic drop to the south. Surface draining appears to be to the south. There are no surface water courses evident near the site. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a dedicated gravel-paved access road which begins after initially sharing the driveway with the house at 9372 Avery Rd. . The access road is currently in generally good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are moderately wooded with single-family residential homes bordering to the north and south but not in the line of sight, and dense

Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surrounding the well and battery and found no signs of impact to the natural greenscape.
