0008293739 2015-05-05 UrbanProduction Wells Violations Noted I inspected this well location at the request of the North Royalton Mayor. The well is located in a clearing within a primarily wooded area at the terminus of Mercury Drive. The entry to the well is gated with a cable-style gate which is locked. The access road is gravel-paved and accesses two wellheads (Callas 1 and 3D). At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of 4 140-barrel steel production tanks, 2 50-barrel poly brine tanks, and 2 mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. ID signage is needed at the entry gate. Evergreen screening is stressed and root balls are exposed in places. A discarded sump pump was observed on the north side of the battery. There is some oil leakage at the stuffing box. Signage is present at the battery but lacking complete information. Signage at the wellhead is falling off. The 8.625-inch casing is not exposed for monitoring as required. Gravel material from the lease road is being dragged onto the roadway by well maintenance traffic and requires management. In addition, the following safety issues were noted. Load lines are not locked. Production tanks are missing lightning arrestors. The dike drain has been left open. These safety issues must be remedied as soon as practicable or by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this matter.
0010369042 2015-02-10 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located at the terminus of a short lease road which begins at the end of Foxwood Dr. There is a short dedicated access road which travel west from the end of Foxwood Drive, and curves slightly northward. The Callas 1 (#2-2085) is co-located with the Callas 3D (#2-2163) and share a common tank battery. The wellhead pump jack and battery are located in separate board-on-board enclosures. The enclosure gates are locked and signage is present. Observable fittings and connections are secure and free from signs of leakage. At the time of this inspection there is 12 to 15 inches of snow cover which limits my ability to inspect the condition of the lease road, interior of the battery containment, and other surficial features.

0311249453 2017-04-27 UrbanProduction Wells No Violations I was at the producing well location to witness on-going remedial efforts following an uncontrolled release of oil that was occurred some time between 04/21/2017 and 04/25/2017. At the time of my inspection I observed the following: crews from Cutter Oil Co. (Cutter) were continuing recovery and mitigation efforts. A twenty-thousand gallon frack tank was delivered and staged near the battery and was to be utilized for collection and temporary storage of recovered fluids from the battery during recovery efforts. Fresh water was delivered via a oilfield tank truck and crews began flooding the battery interior and recovering, with vac truck, the resultant fluids (oil and water) from the existing drain sump. These efforts continued throughout the day, and would continue until all available free oil had been recovered. I walked the perimeter of the pond and found some small areas of partially degraded, residual, floating free-phase oil. I met with Cutter crews and they worked to recover the observed areas of oil on the pond. In the afternoon, Cutters crews excavated an exploratory trench along the pathway of the secondary containment drain. Upon inspection, we observed the 4-inch PVC drain line was connected to a corrugated, perforated drain tile. The connection point was approximately 10 feet from the outside exterior battery wall. Based on this assessment, it appeared that oil was escaping the drain fitting connection in side the battery drain sump, and then entering the subsurface at the connection between the PVC and corrugated pipe. We agreed that the best path forward was to remove the drain which would eliminate the migration pathway. By the conclusion of the day, Cutter crews had removed the drain, with a short section capped and abandoned in place. This area was to be left open overnight for continued assessment.
0516621988 2015-08-24 UrbanProduction Wells No Violations I inspected this producing unit as a follow-up to an earlier inspection in which several safety deficiencies had been noted. Those deficiencies consisted of the following: unsecured tank lids, unlocked load-lines and electrical boxes, incomplete or damaged ID signage, open containment drain, and missing spark arrestors on the production tanks. At the time of this inspection, the noted safety deficiencies have been addressed. The previous inspection had also noted some oil leakage at the stuffing box fitting, and some discarded equipment and debris in and around the battery. At the time of this inspection, these issues have been addressed. I have also discussed with the owner the on-going management of loose gravel at the head of the lease road which had been washing into the local storm sewer. The owner is aware that the gravel will necessarily need to be maintained.

0954790906 2017-05-01 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a release of oil that was discovered on 04/25/2017. At the time of discovery, it was estimated that 20 barrels of oil was released from two of the four 140-barrel oil storage tanks located in the battery. The cause of the release was from a failure of a brass union fitting along the cross-over line. The cause of the failure had not been determined. The oil was contained with the containment dike with the exception of a small amount (less than 20 gallons, estimated) of oil that escaped through the dike drain structure and migrated to the storm water retention pond located adjacent to the battery. Remediation and restoration activities were commenced on the day of discovery and have been on-going. At the time of this inspection, the 140-barrel tanks had been returned to the battery. Clean, course-grained ballast material had been delivered and placed in the battery. I walked the perimeter of the pond and found no signs of residual oil with the following exception: there was a small area of sheen on the surface, on the west side of the pond. I pointed out the sheen to the Cutter crews and advised that this area should be monitored. The area of the hard boom at the outfall was secure and free from residual oil.

At the time of this inspection, I met with EOS Energy Representative Mike Corp and North Royalton Fire Chief Chegan. I noted that the well was in active production. The enclosure gates were locked and secure. The identification was current and accurate. The load-line valves were plugged and locked. Tank lids were closed and sealed. I walked the areas surrounding the well and battery and found no signs of problems or indications of impact. I noted the following maintenance items that required attention: I noted that there was a 250-gallon plastic tote-style container which was not properly labeled. I advised Mike to have these items remedied as soon as possible. I also noted that the gate at the access road was not secure and required proper signage. He indicated that he would have the issues addressed as soon as possible.

1416646850 2012-11-30 UrbanPreliminary Restoration No Violations My visit to the well today was to perform an Urban Preliminary Restoration Inspection. The access road is crushed limestone packed and solid to the well site. The wellhead is enclosed inside board on board fencing and is neat, clean and free of debris. The tank battery is a shared location with the Callis 3D well, Permit #2-2163. This well has two 140 BBL storage tanks and caps are closed and secure. A vent system is installed for the tanks and a charcoal filtering system is in place. There is one horizontal and one vertical separator in the unit. Pick up lines are located inside the gate and they are secured by locks and bull plugs. There is one 35 BBL brine tank. A metering run is in place. Landscaping and restoration is complete. No violations were found and Urban Preliminary Restoration Inspection Passed.
1451531747 2017-04-25 UrbanProduction Wells Violations Noted I responded to this well location pursuant to a report from the ODNR emergency response operations center of an uncontrolled release of oil. On my arrival, I met with crews from Cutter Oil Co. (Cutter), Fred Romeo (ODNR Emergency Response), Wade Balser (OEPA), Thomas Habak (N. Royalton Asst. Fire Chief), Nick Cinquepalmi (Service Director), and Mark Schmitzer (City Engineer). Based on our discussions, the following had occurred: an estimated 20 barrels of crude oil escaped from a 4-inch cross-over fitting connection in the battery. The connection failed and became decoupled for unknown reasons. An estimated less than 20 gallons of oil escaped the secondary containment from a point on the north side of the containment dike and entered the adjacent storm water retention pond. An initial evaluation had indicated that the oil was restricted to the limits of the pond and was not observed to have migrated beyond the pond via the outfall structure located at the northeastern quadrant of the ;pond. Initial response by the N. Royalton FD and Cutter Oil included placement of 200 feet of oil sorbent booms and pad material in the pond to contain and begin recovering the free-phase oil. During my inspection, mitigation and recovery work continued and included the following: Wade Basler deployed a hard boom along with a sorbent boom at the outfall. Cutters crews continued collected free oil form the surface water. Cutter brought in a vac truck and began recovering free oil from the battery interior. Crews also excavated a shallow cut-off trench immediately down-gradient of the battery drain outfall, and constructed a temporary weir dam to prevent any additional oil seepage from entering the pond. A discussion with Cutter indicates that the last visit to the well location by a tender/pumper was on Friday 04/21/2017. So the initial occurrence of the leak was some time between 04/21/2017 and 04/25/2017. By the end of the day, most of the recoverable free phase oil and been removed
1454685589 2016-12-10 UrbanProduction Wells No Violations I inspected this well location pursuant to a call from North Royalton Mayor Robert Stefanik. The Mayor reported that there had been a repot of a loud noise coming from the area of the Callas 1/Callas 3D well pad. I, in turn, contacted CJ Cutter and advised him about the complaint. He was responding to the location. Upon my arrival I met with Fire Chief Chegan and CJ Cutter. The apparent cause of the noise was related to the small compressor unit inside the tank battery. The system had been shut down at the time of my arrival and work was occurring at the compressor. During my visit, I inspected the other elements of the unit. Fittings and connections at the wellhead were secure. The enclosure gate was locked. Load-lines are locked and plugged. Hatch lids are closed and sealed. The secondary containment is intact and dry. The dike drain is secure. By 11 AM, the compressor had been serviced and placed back into production. I contacted the Mayor and advised him that the cause of the noise had been identified and fixed.
1454944844 2016-02-08 UrbanProduction Wells No Violations I inspected this well as a follow-up to a reported complaint on Friday (02/06/2016). At the time of the complaint, an unidentified noise was reported to the local Fire Department. Upon my inspection on Friday, it was discovered that the cause of the noise was ice accumulation in a compressor unit. Repairs were made by CJ Cutter and the compressor unit was returned to service. At the time of this inspection, the compressor is running and free from signs of abnormal operation.
1489046667 2012-06-19 UrbanProduction Wells No Violations My visit to this well today was to meet with C.J. Cutter, owner Cutter Oil. In an attempt to control several events of gas vapor being released into the atmosphere, Cutter Oil is making changes in the routing of lines at this well. The company is plumbing the wells to vent from the compressor pop off valve back into the gas separator unit. Any pressure exceeding 500 PSI will then be vented into the storage tank. This process should resolve the problem of excess gas vapors being released into the atmosphere.
1493918072 2017-05-04 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a release of oil that was discovered on 04/25/17. Since the time of the discovery, remedial and recovery efforts had been on-going. At the time of this inspection, I met with Wade Balser (OEPA) to review and document the remedial progress. I observed the following: the tank battery interior had been restored with newly-imported gravel ballast material. The tanks had been returned to the battery and the connections restored. The secondary containment drain had been removed and the former location of the drain had been filled in with site-sourced compacted clay. The storm water retention pond was free from oil, with the exception of a small area of floating pond scum and degraded oil. Mr. Balser removed the hard boom at the pond outfall and indicated that the OEPA was satisfied and considered the remedial response complete. I advised CJ Cutter about the need to skim the north area of the pond where we had observed the residual oily scum.
1494336219 2017-05-08 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previous uncontrolled of release of oil that had been discovered on 04/25/16. At the time of the discovery, it had been estimated that 20 barrels of oil had escaped from the storage tanks into the secondary containment. An estimated 20 gallons of oil migrated beyond the secondary containment and into the adjacent storm water pond. Remedial measures were initially implemented by the North Royalton Fire Department and subsequent remediation and restoration was implemented and completed by Cutter Oil Co. At the time of this inspection, the impacted areas have been remediated and restored. The well has been returned to production. I had met previously with Wade Balser from OEPA Division of Emergency and Remedial Response and he had indicated that the OEPA is satisfied that remediation is complete.
1648723992 2017-04-28 UrbanProduction Wells No Violations I was at this well location to witness on-going remediation activity following an uncontrolled release of oil that was discovered on 04/25/2017. At the time of ths inspection, crews from Cutter Oil Co, (Cutter) were continuing remedial efforts in the battery and beginning to dismantle the battery enclosure, and disable tank connections ahead of removal of the tanks and separators. Over the course of the day crews completed the removal of the tanks and separators and removed the impacted gravel ballast and soils. Residual oil was recovered to the maximum extent practicable and peat sorb was applied to remainder. Clean ballast material had been delivered previously and was loaded into the battery containment area. I walked the pond perimeter and did not observe any signs of residual oil. The hard boom that had been placed previously by Wade Balser (OEPA) was secure, and free from any signs of oil. I met with CJ Cutter and discussed the planned activity moving forward. He indicated that crews would return to the location on Saturday (04/29/2017) to continue restoration work at the battery. We agreed to meet again at the site on Monday (05/01/2017) and review site conditions.

1954467238 2017-04-26 UrbanProduction Wells No Violations I was at this well location to witness on-going remedial activity following an uncontrolled release of oil that was discovered on 04/25/2017. At the time of this inspection, crews from Cutter Oil Co. (Cutter) were continuing efforts to recover residual oil from the pond, and remove free phase oil from the tank battery interior. I met with CJ Cutter and we agreed that the goal for remediation must include the following elements: removal of oil from the pond, removal of free oil, oil-impacted gravel ballast and soil to the maximum extent practicable, investigate and eliminate the migration pathway for oil escaping the secondary containment, restore the impacted areas. Wade Blaser (OEPA) had indicated that he will leave the hard boom from the outfall area once the free oil has been removed and a post-assessment indicates no further threat to down-gradient receptors exists. I met with Mayor Robert Stefanik and we discussed on-going remedial efforts as well as remedial goals and expectations. Also, pursuant to a discussion with Mayor Stefanik, I met with City Engineer Mark Schmitzer regarding a drainage swale that traverses the property along the east side of the lease road. Mayor Stefanik had inquired whether Cutter could assist with some persistent ponding that had been occurring along the drainage way. Mr. Schmitzer indicated that he would review existing storm water plans that exist for the area and render an opinion whether any alteration was prudent.

I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity. The access road gate had been left open at the request of Cutter Oil due to the snow being plowed into the entrance and making access difficult for the water haulers.
