Theres is a short dedicated access road which travel west from the end of Mercury Drive, and curves slightly northward. The Callas 2 (#2-2084 is co-located with the Zilich (#2-2159) and share a common tank battery. The wellhead pump jack and battery are located in separate board-on-board enclosures. The enclosure gates are locked and signage is present. Observable fittings and connections are secure and free from signs of leakage. At the time of this inspection there is 12 to 15 inches of snow cover which limits my ability to inspect the condition of the lease road, interior of the battery containment, and other surficial features.

At the time of this inspection, I noted the following: a wellhead with pump-jack system and associated tank battery in separate board-on-board enclosures. The battery is shared with the Zilich #2D, located on the same parcel. The enclosure gates were locked and secure. Fittings and connections at the wellhead were secure. The surface casing pressure was being monitored and showed no pressure. The electrical panel box w as locked and secure. Fittings and connections at the battery were secure. The load-line valves were plugged and locked. The tank lids were closed and sealed. The secondary containment drain was secure. The dedicated gravel-paved access road was level and dry. I did note some landscaping fabric debris in the parking area and I notified CJ Cutter about the need for removal of the debris. The entry gate was locked. I walked the areas surrounding the well and battery systems and observed the natural soils and vegetation to be free from signs of leaks, spills, or other adverse impact from the on-going production activity.

At the time of my inspection, the fittings and connections at the well were secure. The tank lids were closed and secure. The load-line valves were plugged and locked. The secondary containment was intact, with some water from recent rains. There was no signs of leakage (floating oil or sheen) visible. I walked and inspected the areas surrounding the wellhead and battery and found no signs or evidence of impacts or degradation. The access point was gated and locked with signage present. The access road was in good condition.

1316437135 2011-09-04 Production Wells No Violations I received a call from CJ Cutter at 17:00 reporting oil on a creek just north of his Butcher well (PN 2180). Cutter checked his wells in the area and could not identify a source. I advised him to contact OEPA-DERR since water had been impacted. I met Cutter at the Butcher tank battery. The oil accumulated behind a debris dam (which acted like a siphon dam) located north of Butcher but appears to have originated upstream. We traced the oil upstream to a storm water drain discharge. The storm drains would need to be checked for impact to identify the source. Cutter called in employees and Moore Well Service to assist in containing and recovering the oil on the creek. Kurt Kollar with OEPA-DERR arrived on site and continued with the recovery and identifying sources which led to the Callas tank battery. Several week ago, oil was discovered outside of this tank battery and was being recovered with sump pumps placed in two recovery points. By this time, darkness had set in and it was agreed that source confirmation and recovery would continue in the morning with excavating and removing impacted material between the wells and the tank battery. A passing thunderstorm assisted in flushing the impacted creek. Product recover will continue into the night. Inspector Lowder will respond in the morning and will issue or extend the Compliance Notice for this well.

Hagen Well Service pulled rods and tubing, replaced pump and reinstalled rods and tubing. Job began at 8:00 Am and was completed at 12:30 PM. ODNR did not witness total operation but was present at beginning and end of project. Cutter Oil to complete construction of protective fence next week and place well back in service.

The battery is comprised of 4 140-barrel steel production tanks, 2 50-barrel brine tanks and 2 separator units with Dominion metering run. The battery is shared with the #2159 unit. Both the wellhead and battery are enclosed in separate board-on-board fences. The gates and load-outs are locked. ID placard and secondary containment are good. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) is present. No problems noted.
1431609213 2015-05-06 UrbanProduction Wells Violations Noted I inspected the Callas # 2 on 5/6/2015. The well commenced production on 2/10/2009 and is current in reporting production to RBDMS. The access road is off of Mercury Rd. and has a cable gate restricting access. The lease road is graveled and affords adequate access. The required signage at the gate does not comply with 1501-9-9:05 OAC. The well is operated as a electrified pumping well. The well is fenced with a wooden plank fence. There is an earthen dike present. The required signage for the well head, again does not meet 1501-9-9:05 OAC. The door was locked and secure. The electrical control panel was unlocked as required by 1501-9-9:05. There is a chemical injection pump and supply drum located below the Sampsons post which is disconnected and is not being utilized at the time of this inspection. If the equipment will not be functional and essential for the production of the well the Operator shall removed it. The stuffing box had previously leaked and contaminated the ground below. There was not a access to the 8.625 annular space as required in the Special Permit Conditions attached to the drilling permit. Another permit condition was to log the entire wellbore, but the Big Lime interval was absent. There is a five (5) gallon bucket of oil and water located near the casing that needs to either be removed or emptied and covered to prevent accumulation. The tank battery ( shared with Zillich # 2-D ) takes up an area of 50 X 50 and is surrounded by a wooden plank fence and earthen dike. The signage on the tank battery is not compliant. The tank battery is void of the second access door. The equipment for the Callas # 2 included two (2) 140 barrel oil tanks, a 50 barrel poly brine tank, a horizontal mechanical separator, a Dom/ EOG gas measurement station and a electric powered natural gas compressor. The oil and brine load lines and electrical boxes were not locked. The oil storage tanks were equipped with tank lids with seals, but not fastened down to make

Electrical boxes are locked and secure. Process safety gear, including spark arrestor is present. Secondary containment at the well and battery are intact. Oil containers have been removed. The low area of the containment berm have been corrected. Leakage at the brine leakage and oil staining have been corrected and mitigated. Update of signage is in progess. There have been reported complaints that storm drainage from the gravel-paved cul-de-sac at the end of Mercury Drive is causing gravel to wash into the local storm sewer. Reportedly, the cul-de-sac is part of the Callas property. Ive advised Cutter Oil to establish a management solution for the drainage. This work is in progress.
1476241895 2011-08-29 UrbanProduction Wells Violations Noted Phone call from Mayor Stefanik of oil deposits along the northern edge of graveled parking area of tank battery for two wells P#2059 callas and P# 2159 Zillich owned by CJ Cutter Oil Company. Met Fire Chief Mike Fabish at site found two areas 1) approximately 1 wide and 3long of dark stained soil and 2) a small patch of leafs coated with oil. Called company they arrived within the hour to remove these area. Excavated into the gravel approximately 18 the gravel was coated with oil. This material was removed and disposed of properly. With flow line exposed approximately 100-150 gallons of fluid was removed by a vacuum truck and put into the water tank inside the tank battery. The excavation was temporarily backfilled area with bark mulch with a sump pipe installed for access to evacuate any fluids that accumulated in excavation. will monitor site for any additional remediation needed.

I walked and inspected the areas surrounding the well and battery and found no indications of impacts or problems relating to the on-going production activity. The shared-use access road was snow-covered by accessible.

The overflow inspection pipe, located at the northwest corner of the tank battery, remains in place for observation purposes. The dike surrounding the well site has been restored to the requirements of 1501-9-9-05. The monitoring casing, located at 3694 Mercury Drive, remains in place. The sump system, located at the storm sewer in front of 3664 Mercury Drive, remains in place per the agreement between Cutter Oil and the City of North Royalton. Once the equipment and sump drains are removed, ODNR will do a preliminary restoration inspection of the well site per 1501-9-9-05 requirements.
1625518751 2011-09-06 UrbanProduction Wells Violations Noted Met with Kurt Kollar of OEPA he showed me what work was completed on Sunday 09-04-11 and Monday 09-05-11. Then proceed to meeting with City of North Royalton and OEPA to discuss the plans for remediation of the site.( Run a camera into the Storm Water Line along Mercury Drive. and to set up an Oil Separator in on tree lawn between address 3674 and 3664 Mercury Drive and to re-excavation that was excavated on 08-29-11to recheck the flow lines for leaks). Returned to site work started at the graveled parking area of the tank battery. CJ Cutter made a phone called to get a solid material vacuum truck to remove rock ,gravel, soil and fluids from the flow line excavation. Once flow lines were exposed Mr. Cutter performed a pressure test on Flow line used 1000PSI on lines a failure at an elbow was observed no other leaks were observed.

The enclosure gates were locked and secure. The identification was current and accurate. The load-line valves were plugged and locked. Tank lids were closed and sealed. I walked the areas surrounding the well and battery and found no signs of problems or indications of impact. I noted the following maintenance items that required attention: I noted that there were some discarded wiring in the battery enclosure which require removal and disposal. I advised Mike to have these items remedied as soon as possible. I also noted that the gate at the access road was not secure and required proper signage. He indicated that he would have the issues addressed as soon as possible.
