0146560125 2017-03-13 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection. At the time of this inspection I observed the following: a wellhead with pump jack system and associated tank battery in separate enclosures which were locked and secure. The valves and connections at both the well and battery were secure and free from signs of leakage. Process safety equipment was present. ID signage was present and accurate. The areas surrounding the well and battery were comprised of fringe areas of shrubbery and grass on a moderate hillslope that drops off to the west towards the Chagrin River, located 1,000 feet to the west. I walked the surrounding areas and found no signs of impacts or degradation. Access to the well and battery was afforded by a dedicated, gravel-paved access road, with a locked access gate. The access road was sound and well drained with no wash-outs or other erosional deficits.
0335196564 2011-12-27 UrbanPreliminary Restoration No Violations On 12/27/2012, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.

1226155626 2013-02-25 UrbanProduction Wells No Violations The producing unit consists of the following elements: A production wellhead and tank battery. The production wellhead consists of a pumpjack system in a 6 enclosure. The tank battery is comprised of 1 100-barrel (BBL) tank, 10 BBL poly tank, mechanical separator, and associated piping and appurtenances enclosed in an 8 enclosure. The system is equipped with production safety equipment required by 1509:9-9-05. The loadout lines are locked and the secondary containment is good. I did not observe any leaks, releases, or problems.
