0138134223 2017-09-15 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and associated tank battery located on an otherwise undeveloped property. The load-line valves were secure and locked. Tank lids were closed and secure. The secondary containment was intact and dry. I walked the areas surrounding the well and battery and found no signs of impacts or problems.
1118392442 2008-11-25 Drill / Deepen / Reopen No Violations drilling at 3600
1241880923 2012-01-12 UrbanPreliminary Restoration No Violations On 01/12/2012, DNR Inspector Richard Skidmore completed an urban preliminary restoration well inspection for DNR Inspector Ray Wallace. The lease road, tank battery, and well head locations are in compliance with 1501:9-9-05. Inspection passed.
1333847071 2013-08-23 UrbanProduction Wells No Violations The producing unit consists of the following elements: A production wellhead and aboveground storage tank (AST) battery. The production wellhead is fitted with a mechanical plunger lift system and is located 75 feet east of the tank battery. The tank battery system, which is reportedly shared with the #1 and #8, includes 1 100-barrel production tank, a 50-barrel production tank, and a 50-barrel brine tank, mechanical separator and associated appurtenances. The tank battery is overgrown with weeds. Secondary containment is adequate. Tank and well spacing is in accordance with 1501:09. The interior of the tank battery needs to be cleared of week overgrowth. I spoke with Jim Adams, contract pumper for Kleese, about the need for maintenance. He indicated that the issues would be taken care of by the end of the week.
138134223 2017-9-15 0:00: UP No Violations 0.50
1455122007 2008-12-01 Drill / Deepen / Reopen No Violations 50 from total depth
1544701320 2008-12-02 Drill / Deepen / Reopen No Violations rig down for repairs
1994171610 2013-02-19 Preliminary Restoration No Violations The wellhead consists of a mechanical lift unit, not enclosed. The associated tank battery consists of 1 100-barrel (BBL) tank, 2 50 BBL tanks, and a mechanical separator, in a separate, locked enclosure. The spacing is in accordance with the rule. The tank battery dike is adequate. The unit identification is hand written and unclear. I contacted Matt Kleese and advised him about the need for revised identification. The tank battery is shared with the #866 unit. There has been no production reports filed for this unit since 2010. I contacted Maryann at Kleese Dev. Assoc. She indicated that they may have forgotten to file the 2011 production report.
