0991447611 2013-08-06 UrbanPreliminary Restoration No Violations The producing unit is comprised of a production wellhead with pumpjack system . The wellhead has an identification placard. There is oil leaking from the stuffing box. I will bring this to Keith Moores attention tomorrow. Identification is present at the tank battery. The tank battery is comprised of 3 100-barrel production above-ground storage tanks (ASTs) and one 50-barrel brine AST in a board-on-board enclosure. There is a Dominion metering run in the battery. There is secondary containment. The unit spacing meets requirements. There is safety equipment as required under 1501:9-9-05E. The tank battery is shared with the Homestead #1D. The access road is in good condition. I did not observe any problems or violations.

1922272355 2017-02-01 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. At the time of this inspection I observed the following: Access to the well and battery are afforded by an unpaved access road that is shared with the house and barn. The access road and areas in and around the well were mostly snow covered, but were in good condition with no signs of erosional deficits or degradation. The production system was comprised of a pump jack and tank battery in separate board-on-board enclosures located in close proximity. The tank battery was shared with the Homestead (#2-2142) well located on the same parcel. The enclosure gates were closed and locked. The pump jack was in operation with no evidence of problems or leakage. I walked the interior of the tank battery enclosure and inspected the tanks and associated fittings, connections, and pipe runs. I did not observe any signs of leakage or spills. The tank load line valves were plugged and locked. The tank lids were closed. I walked the areas surrounding the wellhead and battery and found no evidence of impact or degradation.
