1354113304 | 2012-11-27 | UrbanPreliminary Restoration | No Violations | My visit to this well today was to complete an Urban Preliminary inspection. The access road consists of a solid based gravel compacted to six inches. The well unit consists of a pump jack unit encased in a board on board fence with locks and chains and is accessible through a gate in the fencing. The tank battery consists of two 140 BBL storage tanks and one 50 BBL plastic brine tank. Tank hatches are closed and secure and vent pipes are in place as is a charcoal filter unit. There is one vertical and one horizontal separator unit. There are two load lines, and two metering runs inside the unit. Load lines are locked and secured with bull head plugs in place. Dyke around tank battery meets standards for urban well requirements outlined in 1501:9-9-05. Tank battery is surrounded by board on board fence and is securely locked. Tank battery is shared by Oyler 2 D, P# 2-2125 and Oyler 3 D, P# 2-2126. Preliminary restoration passed. |
1431547917 | 2015-05-07 | UrbanProduction Wells | Violations Noted | I inspected this well location as part of routine inspection activity. The well is located in a clearing within a primarily wooded area. The entry to the well is gated with a cable-style gate which is locked. . The access road is gravel-paved and accesses each of three wellheads (Oyler 1, 2D, and 3D). At the time of this inspection, there are no wash-outs or erosional deficits. The wellhead is comprised of a pump jack unit in a board-on-board enclosure which is locked. The shared tank battery is comprised of a total of six 140-barrel steel production tanks, three 50-barrel poly brine tanks, and three mechanical separators. The entry door is secure and locked. At the time of this inspection, the following maintenance issues were noted: Electrical boxes are not locked. Production tank lids are not latched down as required. The dike drain riser is not secure and leaking which requires remedy. There is a significant gap between the bottom of the fence enclosure and the ground surface at the well enclosure and battery rendering the enclosures unsecure. Signage is present but lacking complete information. There is some leakage of oil from the polish rod stuffing box, and at the tank load-outs, which requires mitigation. The piping to the adjacent indirect heater must be properly buried. The heater enclosure is unlocked. The 8.625-inch casing is not exposed for monitoring as required. In addition, the following safety issues were noted. Load lines are not locked. Brine is actively leaking from the 2-inch flange at the brine tank. These safety issues must be remedied by 06/19/2015. Please contact Inspector Stephen Tompkins with questions regarding this Compliance Notice. |
1818684442 | 2015-09-03 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previous Compliance Notice. The following violations have been corrected: No locks on the load-lines and leaking brine from the poly brine tank. In addition, the observed gap in the battery containment dike has been remedied. The tank lids are closed and secure. The 8.625-inch casing is plumbed to surface as required. Work in the tank battery is underway and the poly brine tanks are all off line and disconnected at the time of this inspection, |
