0018299553 2017-11-28 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. The well and battery are located on the grounds of the St. Sava Church. The battery is shared with the #2-1981 and #2-1982 wells located on the same parcel. At the time of this inspection, I observed the following: the fittings and connections at the wellhead pump jack system and the battery were secure. The tank lids were closed and secure. The load-lines were secure, plugged and locked. The secondary containment was intact and free from signs of leakage. The enclosure gates were locked and secure. The shared-use access consists of asphalt-paved drive-way and parking for the church. I walked and inspected the areas surrounding the well and battery and found no overt or observable signs of environmental impact. The production reports were current. Bonding and insurance for this owner were current.
0556313309 2013-04-03 UrbanProduction Wells No Violations The producing unit consists of the following elements: a production wellhead and a storage tank battery. The production wellhead is fitted with a pump jack system and is currently producing. The pump jack unit is enclosed in an 8-foot board-on-board enclosure which is locked. The stuffing box has leaked and there is oil surrounding the wellhead. Some absorbent pads have been placed on the ground. The tank battery consists of 4 100-barrel steel tanks, 1 50 barrel tank, and 2 separator units. The battery is enclosed in a chain link enclosure. The secondary containment is present and in good condition. The tank battery is shared with the St. Sava 1 and 2 Units. I called Ralph Gregory with Bass Energy and advised him that the unit needs attention consisting of the following actions: Make repairs to stuffing box, clean-up oil from around the wellhead. He indicated that crews would tend to this unit tomorrow (4/4/2013).
0658336614 2014-02-20 UrbanProduction Wells No Violations The unit is producing with no changes observed since the last inspection. No violations or problems.
1049556588 2011-09-07 UrbanProduction Wells No Violations I inspected this well today as a preliminary restoration inspection. The well is a rabbit well and is encased within a chain link fence structure located on the right side of the driveway as you enter the church parking lot. All gates are secured with locks and the wellhead area is neat and clean. The tank battery is located behind the well and consists of a horizontal separator, two storage tanks and a meter run to the street. There are two pickup lines on each storage tank and they are secured with bullhead plugs in place. The tanks are surrounded by a dike sufficient to meet the standards of 1501-9-9-05. The area of the tank battery is clean and neat and no violations are noted. Preliminary restoration is complete and passed.

1294253989 2015-01-29 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The wellhead pump jack enclosure is secure. Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow. The tank battery which is shared with the #2-1981 and the #2-1982 units is secure. Valves and connections are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.
1441724200 2015-09-08 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located on the west side of the asphalt paved-entry drive to the church grounds. The well is comprised of a pump jack system in a board-on-board enclosure. The gate is locked and secure. Fittings and connections at the wellhead are secure. Signage is present and complete. The battery is located to the immediate south in an adjacent wooded area. The remote load-line connection adjacent to the driveway is locked and secure with a integral clam-shell style containment system. The battery is located in a chain-link enclosure which is locked and secure. Spark arrestors, carbon filters, and low-pressure relief valves are present. Tank lids are closed and latched. Fittings and connections are secure. The tank battery is shared with the #2-1981 and the #2-1982 units, which are also located on the grounds of the church. I performed a reconnaissance of the area surrounding the well and battery. There is a small drainage stream which traverses the area. I did not observe any evidence of problems associated with the production activity. The secondary containment berm is intact and the drain is secure.

1786393772 2019-05-06 Production Wells No Violations I conducted an inspection of the well and found no violation at well or tank battery.
1795371252 2015-05-06 UrbanProduction Wells Violations Noted I inspected the St. Sava # 3 on 5/6/2015. The well commenced production on 5/1/2008. The access road to the well head and tank battery is graveled and affords adequate accessibilty. The signage was present but does not meet the requirements of 1501-9-9: 05 OAC. The well is produced with an electrified pump jack. The well head is surrounded by a wooden plank fence. The access door was locked. The well head was leak free. There was not an access to the 8.625 annular space as required in the permit conditions. The required lock on the electric box was absent. Behind the pumping unit was a gas heater enclosed in a wooden plank fence which was locked and secure. The tank battery ( shared with St. Sava # 2 & # 1 ) for the St. Sava # 3 consisted of two (2) 140 barrel oil tanks, a 50 barrel brine tank, a horizontal separator and DOM/EOG gas measurement station. The tank battery was secured by a chain link fence and locked gate. The tanks had the required lightening arrestors, low pressure tank vents, charcoal filter and hatches. The inspection hatches were not fasten down, limiting its usefullness. The three (3) inch valves for the load lines are leaking and contaminating the ground. The pick up load lines are remotely located away from the tanks and has a plastic containment bowl surrounding the valves. This containment bowl is leaking and causing ground contamination. In order to bring the St. Sava # 3 into compliance the Operator shall replace or repair the leaking valves on the tanks and clean up the ground contamination under the pick up containment bowl. These remedies shall be completed and approved by DOGRM by 6/18/2015. Operator shall place the required locks on the electric box for the pump jack and remove any non estential items ( meter houses ) from the tank battery and pumping unit.
