Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow. The tank battery which is shared with the #2-1981 and the #2-1983 units is secure. Valves and connections are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.

The well is located south of the church, adjacent to a party house and ball field. The battery is located on the west side of the asphalt paved-entry drive to the church grounds. The well is comprised of a pump jack system in a board-on-board enclosure. The gate is locked and secure. Fittings and connections at the wellhead are secure. Signage is present and complete. The battery is located in a wooded area adjacent to the #2-1983 unit. The remote load-line connection adjacent to the driveway is locked and secure with a integral clam-shell style containment system. The battery is located in a chain-link enclosure which is locked and secure. Spark arrestors, carbon filters, and low-pressure relief valves are present. Tank lids are closed and latched. Fittings and connections are secure. The tank battery is shared with the #2-1981 and the #2-1983 units, which are also located on the grounds of the church. I performed a reconnaissance of the area surrounding the well and battery. There is a small drainage stream which traverses the area of the battery. I did not observe any evidence of problems associated with the production activity. The secondary containment berm is intact and the drain is secure. There is a small drainage feature adjacent to the well, and a storm drain located 50 feet to the south. At the time of this inspection, the drain is dry with no signs of problems, oil, or other runoff from the well.

The load-lines were secure, plugged and locked. The secondary containment was intact and free from signs of leakage. The enclosure gates were locked and secure. The shared-use access consists of asphalt-paved drive-way and parking for the church. I walked and inspected the areas surrounding the well and battery and found no overt or observable signs of environmental impact. The production reports were current. Bonding and insurance for this owner were current.
1444851946 2013-04-04 UrbanProduction Wells Violations Noted The producing unit consists of the following elements: a production wellhead and a storage tank battery. The production wellhead is fitted with a pump jack system and is currently producing. The pump jack unit is enclosed in an 8-foot board-on-board enclosure which is locked. The stuffing box has leaked and there is oil surrounding the wellhead. Oil has migrated beyond the enclosure, along a depression, approximately 4 feet. The soil is saturated with oil to an approximate depth of 1 to 2 inches. The tank battery consists of 4 100-barrel steel tanks, 1 50 barrel tank, and 2 separator units. The battery is enclosed in a chain link enclosure. The secondary containment is present and in good condition. The tank battery is shared with the St. Sava 2 and 3 Units. I called Ralph Gregory with Bass Energy and advised him that the unit needs attention consisting of the following actions: Make repairs to stuffing box, clean-up oil from around the wellhead and remove oil-contaminated soil. He indicated that crews would tend to this unit tomorrow (4/4/2013).
