0226947601 2015-09-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located in a residential area on the west side of Chagrin River Road. The area immediately surrounding the well is comprised of a small wooded portion of a larger parcel. Access to the well and battery are afforded by an unpaved, dedicated lease road which enters from Racerbrook Drive and travels 300 feet to the unit. At the time of this inspection, the well is in production and records are current. Enclosure gates are locked and secure. Load-lines are locked and secure. Fittings at the wellhead and battery are secure and free from leakage. Battery secondary containment is intact and free from staining or other problems. ID signage is present and accurate. The access road is in good condition and well drained. Areas surrounding the wellhead and battery are free from signs of impact relating to production activity.

0510876155 2017-11-20 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. At the time of this inspection the wellhead and battery board-on-board enclosures were closed and locked. The load-line valves were secure and locked. Tank lids were closed and secure. Secondary containment as intact and free from signs of release, leaks, or problems. The well was in active production. Production reports were current. Bonding and insurance were active and current. I walked and inspected the areas surrounding the wellhead and battery and found no evidence of impacts from leaks, releases, or degradation to the natural greenscape.

1403108754 2014-06-18 UrbanProduction Wells No Violations I inspected this producing unit as part of regular site check and inspection activity. The well was completed in 11/24/2006. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. The 8.625-inch casing is valved to surface but is not gauged. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located southeast and adjacent to the wellhead. The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel poly brine tank, and a horizontal separator. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area of the well is generally level with a rapid topographic drop to the south and east. Surface draining appears to be towards an unnamed tributary to the Chagrin River which lies approximately 100 feet to the southeast. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a dedicated gravel-paved access road which is partially shared with other homes along Racerbrook Drive. The short access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are heavily wooded with one single-family residential home bordering to the northwest, and no other residential strucutres within eyesight.
