0394106338 2009-07-16 Production Wells No Violations The Uniontown office received a complaint from Mr. Tom Hurley (216-554-0558) at 6936 Farview Drive in Brecksville. Mr. Hurley reported milky brown discoloration and an odor from a creek which crosses the south side of his property. The wells at St. Michael are located upstream from Mr. Hurley. The inspections found no evidence of leaking or discharge from either the wells or the tank batteries. There was however, evidence of dumping on the east end of the St Michaels parking lot. A storm drain located next to the surface staining, contained discolored water. A discharge pipe from the storm drain empties into a ravine/tributary to Mr. Hurleys creek. This drain contains the first sign of any change in the water. Since there was no evidence of any involvement with the oil and gas wells, I contacted Kurt Kollar with OEPA-DERR to report the release and assume the investigation.

The wellhead and battery are located behind the St. Michaels Church on Mill Road just south of Wallings Road. The head of the dedicated lease road is located at the rear parking area and is gated. At the time of this inspection the gravel-paved lease road is in good condition with no drainage or erosional deficits. The wellhead and battery are located in separate chain-link enclosures and are locked. Connections and fittings at the wellhead and battery are secure and free from signs of leakage or other problems requiring attention.

Secondary containment was intact and secure. The individual enclosures were locked and secure. The access road was well drained with no signs of wash-outs or erosional deficits. ID signage was present.

The enclosures are secured with locks and in in good condition. The tank battery is comprised of two 140-barrel steel tanks, and mechanical separator. HB 278 safety equipment is in place. Secondary containment is present and adequate. I did not observe any problems or violations.
