1323521851 2013-01-07 UrbanProduction Wells No Violations The producing unit is comprised of a two-tank battery (2 210-barrel) with mechanical separator and a wellhead with mechanical plunger lift. The tank battery is not enclosed. The wellhead is enclosed in a 6-foot, chain-link fence. The well is currently in production. The tank and well spacing is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate. I did not observe any leaks, spills, or other signs of contamination.
1401393701 2014-05-29 Plug / Plug Back No Violations I inspected and observed the plugging and abandonment activities being performed by Nicks Well Plugging, as contractor to Duck Creek Energy. The plugging activity is being performed at the discretion of Duck Creek based on a lack of satisfactory production. All activities associated with plugging and abandonment were performed without incident and are documented in the associated plugging report dated 05/29/2014.
1606011951 2017-10-20 Final Restoration No Violations I inspected this well location as an evaluation for Final Restoration. Plugging and abandonment was completed on 5/30/2014, witnessed by a ODNR Division of Oil and Gas Inspector, and a plugging and abandonment report was filed. At the time of this inspection, the former location of the well had been regraded, and restored to match the surrounding area conditions. The former battery location had been restored. The former production elements, including the flow-line, were removed and there was no remaining system elements. No further action is needed. Final Restoration is passed.
