1365696490 2012-09-13 Orphan No Violations My visit to this well on this date was at the request of Norburt Lowder, Mineral Resources Inspector 3. I was on site to witness the excavation of a portion of an existing 18 x 18 x 3 deep poured cement vault which is leaking. There is wellhead located in the center of the vault. The wellhead has a cap with a 2 vent pipe which extends to a vent located in the Southwest corner of the property. The vault is to be excavated is approximately 8 in diameter by 6 deep through the bottom of this existing vault. A new, four section, 6 diameter x 5 deep circular vault is to be placed around the wellhead and backfilled on the outside of the new vault. The new vault was set into place at 10 below grade and two (2) 3 galvanized vent pipes were plumbed and installed. There was one vent to the vault and one to the wellhead. They were placed in the southwest corner of the property and covered to grade with gravel and dirt. There are test holes in the new vent pipes which are capped and secure. Work on the project is to be completed on September 14, 2012.

1437751157 2015-07-23 Orphan No Violations I inspected this reported well location as an orphan well status update. The landowner and complainant, Mr. Rulong, indicates a subcontractor, hired under Orphan Well Program contract, performed work on this well within the last 2 or 3 years. These activities consisted of the construction of a new passive vault and vent system. At the time of my inspection, the well location is not vissible or apparemt. There is a vent stack near the southwest property border. The vent stack is comprised of a pair of 2-inch diamater steel stack risers that terminate 12 feet above grade. The area where the well is located in the back yard and is covered in turf grass. The small lot size and location of the well will likely preclude access with a service rig.

1530816477 2012-09-14 Orphan No Violations On my visit to this well today I was accompanied by Richard Bayless, Training co-coordinator for DOGRM. The contractor on the project, R.A. Gibson Associates was in the process of hooking up all vents to the well head and the vault. Following these procedures, the top cap was placed on the vault and the area was filled with topsoil, seeded and covered with straw. I considered the project to be completed according to contract specification, and preliminary restoration passed.

2121740328 2012-10-16 Orphan No Violations My visit to this location today was at the request of Mr. Ron Gibson of R. A. Gibson Associates. Mr. Gibson informed me that Mr. Mike Rulon, owner of this property was unhappy with the results of the grass planting on his property. Mr. Gibson requested that I meet him at the property located at 4342 West 155th street. I met Mr. Gibson there and we had a discussion concerning the matter, and Mr. Gibson and Mr. Rulon met privately for a few minutes. Following that meeting Mr. Gibson informed me that he and Mr. Rulon had come to an agreement on the issue. A waiver was signed by both parties concerning additional top soil and seeding of the lawn. Mr. Rulon agreed to do the work himself at a later date. A copy of the waiver is attached to this report.
