0765781761 2015-05-06 UrbanProduction Wells No Violations I was on site to conduct a status check of this well with DOGRM Inspector - John Fleming. The wellhead was the same since last report. It was observed that the tank battery had been recently updated to include a new flow line and consisted of 1- 140 bbl production tank, 1- 100 bbl, production tank, mechanical separator, and a 1500 gallon poly brine tank. The dike, signage, access were good. No violations found.
0935115385 2013-07-31 UrbanProduction Wells No Violations The producing unit consists of the following elements: a wellhead with pumpjack, and a tank battery system. There is no ID at the wellhead. The wellhead and tank battery are in separate chain-link enclosures. The tank battery is comprised of two 140-barrel steel production tanks and one 25-barrel poly tank, mechanical separator, and metering run. Secondary containment is present at the tank battery. Unit spacing is in accordance with the regulations. I observed a significant amount of oil staining and oil impacted material around the mechanical separator. The bottom of the separator appears to be failing or leaking. The tank battery is shared with the #201 unit. I contacted Oxford Oil about the need for mitigation of oil release at the tank battery.
0973994187 2017-01-19 UrbanProduction Wells Violations Noted I inspected this well as part of routine inspection activity. At the time of this inspection, I observed the following: a wellhead with pump jack system with associated tank battery in separate chain-link enclosures. Fittings and connections at the wellhead and battery were secure and free from signs of leakage or problems. The secondary containment was intact and free from signs of spills or leaks. I noted the following maintenance deficiency: the transfer of ownership of the well was completed on 08/02/2016. The existing ID signage and emergency contact information was not current and reflected the previous owner. M & R Investments Ohio, LLC must update this signage with current and complete information as required by OAC 1501:9-9-05(E)(1). This must be executed by 02/24/2017. Contact Inspector Stephen Tompkins with any questions about this compliance notice.
1827466677 2017-03-10 UrbanProduction Wells No Violations I inspected this well location, as a follow-up to a previous compliance notice issued for lack of correct and complete identifying signage. At the time of this inspection, the ID signage has been updated and is correct and complete. I walked the areas around the wellhead and battery and found the fittings and connections secure. The access to the well is afforded by a shared-use, gravel-paved access road which enters from Ridge Road and traverses the farmstead. The well and battery are located on a ridge line that drops off to the east towards an unnamed drainage feature. The battery is shared with the Zajicek #1 (#2-0201) well located beyond the 900 feet to the east, in a small clearing within a wooded area. The access road was as in good condition with no significant drainage deficits or other problems.
1883982492 2019-04-24 UrbanProduction Wells No Violations At the time of my inspection the well was in production. The well is equipped with a pump jack unite. The production valve was open at well. The tank battery consisted of two 100-barrel production tanks, one 50-barrel production tank, and a mechanical separator. The load line to the tanks production valve was secured with a plug and a lock. An earthen dike surrounded the tank battery. Proper identification was posted at the tank battery. The lease road was in stable condition.
