0107283135 2014-02-11 Drill / Deepen / Reopen No Violations On the day of my inspection I met with Rose Peti at 36246 CR 70, West Bedford OH, Coshocton Co. Pike Twp. The ground was snow covered at the time of inspection. I found no evidence that any type of fluids had been dumped, spilled, or otherwise disposed of in or around the culvert in question. There were no areas of snow melt and there were no significant ice formations in the area. I did uncover some ice at the base of the culvert. I broke the ice up and placed a few chunks in a plastic field sample collection bottle. I placed the bottle on the defrost vents of my truck and melted the ice. I conducted field tests of the waster for pH and chloride concentration. The pH strip indicated 6 and the Quantab indicated 28 ppm (parts per million). The results were indicative of typical roadside drainage. I contacted Bill Mitton to request that he instruct his contractors not to park in front of her house for the remainder of the drilling.
0251823681 2014-02-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Kilbarger Drilling Rig 5 was drilling the 7.875 production hole on air, at a depth of 1674. The permit was posted and I found no leaks or spills around the location. I conducted a visual inspection of the area around the lease road entrance and surrounding properties in response to a notification from the Coshocton Co. Health Inspectors Office that a complaint had been called in about illegal dumping of hazardous chemicals. The ground was snow covered and I found no signs of a fluid release near in the area.
0290786650 2014-02-12 Casing String Witnessed No Violations On the day of my inspection I found that Kilbarger Drilling rig 5 ran used, tested 4.5 casing to 3442. Petroset cemented the casing with 100 sacks of class A cement.

The site was approximately 30% covered in new grass growth. I found no drilling materials or debris. This site does not pass preliminary restoration at this time.

Health Inspectors Office to inform me that he received a complaint from Rose Peti in regards to illegal dumping of hazardous chemicals into her culvert. Mr. Lonsinger explained that Ms. Peti claimed that there was suspicious activity in front of her house at 03:30 on 2/9/14. I notified Mr. Lonsinger that Knox Energy was drilling a well in the area and I was aware that Petroset was on location to cement surface casing at the approximate time of the claim. Mr. Lonsinger claimed that he would check the area and contact me if he found any signs of fluid release.
1392463461 2014-02-11 Administrative Inspection No Violations I contacted Steve Lonsinger of the Coshocton Co. Health Inspectors Office to follow up on his field investigation. He claimed that he found no evidence that anything was being illegally dumped in the culvert on Rose Petis property.

1469023680 2014-02-11 Administrative Inspection No Violations I received a phone call from Rose Peti, who claimed that she witnessed fracking trucks dumping fracking water into her culvert at 03:30 on Sunday morning 2/9/14. I informed her that Knox Energy was drilling a well in the area and I was aware that Petroset was on location to cement surface casing at the approximate time of the claim. I agreed to meet her at the site of the alleged infraction to conduct a more thorough inspection.
