0373099586 2015-06-01 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The motor was intact and the valves were open. The tank battery consisted of one 210 barrel tank, one separator and an earthen dike. Identification was posted. I found oily soil around the wellhead. I contacted Mark Fluharty, pumping contractor for Northwood, to get it cleaned up.

1298461368 2015-06-15 Production Wells Violations Noted On the day of my inspection I found that there had been work done to clean up the area around the wellhead, but the well was still creating a mess. The well was pumping during my inspection, and there was fluid spraying out of the stuffing box on every down stroke. I called Mark Fluharty and he met me at the well with the pumper. They tightened the stuffing box to stop the leak. I told Mark that they still needed to remove the contaminated soil around the wellhead.

1456505315 2016-02-22 Production Wells No Violations Regional Supervisor Tom Hill and I were at the site to investigate Complaint #5264. The lease road to the well and tank battery was in good repair and stable. The well is produced with an electrically operated pumpjack. Production valves were open. At the wellhead, there was evidence of a prior spill that had been cleaned up. There were a number of egg-to-fist sized amounts of oil-stained soil within approximately ten feet of the wellhead. The oil was not mobile. Gravel had been scraped from the drive in front of the tank pad and deposited around the wellhead. The wellhead plumbing had been wiped down, but there was some residual oil remaining. The surface casing head was below grade and not accessible. There is a domestic gas connection at the wellhead. Next to the pumpjack pad, there is a pile of tubing and rods that needs to be removed. Proper identification is posted on the production tank. The identification sign is peeling and weathered and should be replaced. The tank battery consists of a single 210--inch diameter sales line. The valves on both the oil and brine pickup lines were secured with plugs. There is a dike present and it is in good repair. The state database shows that in 2014, production was reported to be 977 MCF gas and 31 bbl brine. There are no violations for this well.

2111061293 2015-07-14 Production Wells No Violations On the day of my inspection I found that the contaminated soil had been removed and replaced with new, clean soil. The leak had been stopped and I found no further contamination. This violation is resolved.
