1412873340 2014-10-09 Production Wells No Violations I met Trevor Irwin of the Ohio EPA at the spill site to inspect the progress of the clean up. Trevor assumed control of the clean up. He coordinated with Scott Kees in the placement of containment booms and clean up operations. NGO was also running plastic liner in the flow line at the time of my inspection. NGO was ordered to leave the containment in place through the weekend, and monitor it periodically. I will be in contact with Scott Kees and Trevor Irwin throughout the duration of the clean up.
1413981836 2014-10-22 Production Wells No Violations On the day of my inspection I found that the containment booms and straw bails had been removed. I did not observe any residual sheen on the water. I did not observe any impacted wildlife or vegetation. I contacted Trevor Irwin, of the OH EPA, to report my findings. Scott Kees, of NGO, reported that they removed the containment on 10/21/14.
1550457465 2014-10-14 Production Wells No Violations On the day of my inspection I found that the plastic skirted boom was still in place at the bridge on CR 103, and the absorbent boom was still in place at the bridge on TR 252. NGO had placed straw bails at the TR 252 containment site. There was still some oil skim collecting at both containment locations and I still observed an intermittent sheen. I contacted Scott Kees, with NGO, who reported that they were still monitoring the booms and removing the skim with their vacuum truck as needed. I contacted Trevor Irwin, with the Ohio EPA, who requested that they leave the booms in place for at least another week and to continue monitoring.

1745477130 2014-10-08 Production Wells Violations Noted I received notification from NGO of a ruptured flow line that resulted in crude oil getting into a creek. When I arrived on location NGO had a clean up crew using absorbent booms and pads to isolate the release. They were also using a vacuum truck to remove the skim from the surface of the water. The flow line is routed under Evans Creek and the rupture occurred under water. They claimed that the well was not pumping and the only oil released was from the 2 flow line from the point of rupture to the well head. I calculated that to be 1.9 barrels. I observed no actual pockets of crude oil. The only observable contamination was a sheen on the surface. I also observed no oil in the creek side vegetation. NGO shut the well in and clamped the flow line. We traced the sheen downstream approximately 2 miles to the bridge at CR 252 and SR 751. I did not observe any sheen further south from that point. I reported the spill to the Ohio EPA and was contacted by Trevor Irwin. NGO was to continue clean up efforts and I will meet Trevor at the location on 10/9/14 to further assess the situation.
